摘  要:目的通过研究外阴尖锐湿疣及宫颈糜烂与高危型人乳头状瘤病毒(HPV)亚临床感染(SPI)的关系,为外阴尖锐湿疣与宫颈糜烂患者SPI感染的预防提供依据。方法回顾性分析2006年7月-2012年7月100例外阴尖锐湿疣及宫颈糜烂患者的临床资料以及55例正常女性的临床资料,其中外阴尖锐湿疣58例,宫颈糜烂42例,对3组患者的基本情况进行问卷调查;经过宫颈液基细胞检查、阴道镜检查以及阴道镜下取组织进行病理学诊断,判断是否存在SPI感染,并经过杂交捕获试验Ⅱ对高危型HPV-DNA感染加以检测。结果 HPV SPI外阴尖锐湿疣组检出29例,感染率为50.00%;宫颈糜烂组检出12例,检出率为28.75%,二者差异有统计学意义(P<0.05);外阴尖锐湿疣组与宫颈糜烂组高危HPV-DNA检出24例与13例,检出率分别为41.38%与22.41%,二者差异无统计学意义,二者明显高于正常对照组(9.09%),差异有统计学意义(P<0.01)。结论外阴尖锐湿疣及宫颈糜烂患者是高危型HPV SPI的高危人群,而高危型HPV SPI患者HPV-DNA的临床检出率明显高于非HPV SPI患者,应该对其进行正常的随访与管理。OBJECTIVE To study the relationship between condyloma acuminate and cervical erosion to the subclinical infection (SPI) of high-risk human papilloma virus (HPV) so as to provide evidence for the prevention of SPI to the patients with condyloma acuminate and cervical erosion. METHODS The clinical data of 100 cases of patients with condyloma acuminate and cervical erosion were retrospectively analyzed from Jul. 2006 to Jul. 2012, 55 cases of clinical data from health women were analyzed as well. Among the 100 patients, 58 cases were with condyloma acuminate, 42 cases with cervical erosion, and the two groups with the 55 health women group were investigated by questionnaire about the basic informatiom liquid-based cervical cytology screening, colposcopy and tissue pathology diagnosis under colposcopy were used to determine whether there was SPI infection, and hybrid capture II was used to detect high-risk HPV DNA infection. RESULTS The HPV SPI oecured in 29 cases of the condyloma acuminate group with the infection rate of 50.0%, which occured in 12 cases of the cervical erosion group with the infection rate of 28. 75%. There was a significant difference between the two groups (P〈0.05). The high-risk HPV DNA was detected in condyloma acuminate group (24 cases, 41. 38%) and the cervical erosion group ( 13 cases, 22.41%), and there was no significant difference between the two groups, while both were significantly higher than the control group (9. 09%) , and there was a statistically significant difference (P〈0.01). CONCLUSION Condyloma acuminate and cervical erosion patients are the population at high risk of HPV SPI, and the detection rate of HPV DNA is significantly higher in the high-risk HPV SPI patients than in the non-HPV SPI patients, it is necessary to conduct the normal follow-up and management.
