妇科腹腔镜手术并发症的临床分析及预防措施

摘　　要：目的分析妇科腹腔镜手术患者并发症发生情况,探讨并发症相应的预防措施,以降低并发症的发生。方法回顾性分析医院2010年3月-2012年12月择期行妇科腹腔镜手术患者的临床资料,调查分析并发症发生情况,采用SPSS 13.0统计软件对研究资料分析并发症与手术类别的关系。结果 1526例患者中共19例出现并发症,发生率为1.25%;子宫切除术并发症发生率为2.4%,子宫肌瘤剔除术并发症发生率为3.1%,两种手术并发症发生率比较,差异无统计学意义;附件手术并发症发生率(0.3%)与子宫切除术和子宫肌瘤剔除术并发症发生率比较,差异有统计学意义(χ2=9.042,P<0.01;χ2=11.207,P<0.01);子宫切除术和子宫肌瘤剔除术患者出血发生率与附件手术出血发生率比较,差异无统计学意义。结论正确认识腹腔镜并发症预防感染,严格按规范仔细操作,提高手术质量,减少并发症发生,保证手术安全。OBJECTIVE To analyze the incidence of complications due to the laparoscopic surgery in department of gynecology and put forward corresponding prevention measures so as to reduce the incidence of complications. METHODS The clinical data of the patients who underwent selective laparoscopic surgery in the department of gynecology from Mar 2010 to Dec 2012 were retrospectively analyzed, then the incidence of complications was investigated, and the study data involving the correlation between the complications and the type of surgery were statistically analyzed with the use of SPSS13.0 software. RESULTS Of totally 1526 patients investigated, the complications occurred in 19 cases with the incidence rate of 1.25%; the difference in the incidence of complications between the hysterectomy （2. 4%） and the hysteromyomectomy （3. 1%） was not statistically significant; the difference in the incidence of complications between the appendix surgery（0.3 %）, hysterectomy, and hysteromyomectomy was statistically significant （X2= 9. 042, P〈0.01 ; X2 = 11. 207, P〈0. 01） ; the difference in the rate of hemorrhage between the hysterectomy, hysteromyomectomy, and appendix surgery was not statisticalIy significant. CONCLUSION Correct understanding of the complications due to the laparoscopic surgery and rigorous surgical procedures can improve the quality of surgery, reduce the incidence of complications, and ensure the surgical safety.
