摘 要:目的 探讨黏附分子CD44与呼吸道合胞病毒(RSV)致毛细支气管炎的相关性。方法 采用流式细胞仪检测47例RSV感染致毛细支气管炎患儿(RSV感染致毛细支气管炎组)、33例RSV感染非毛细支气管炎患儿(RSV感染非毛细支气管炎组)及40例健康儿童(健康对照组)外周血 CD44+淋巴细胞占总淋巴细胞的比例,用ELISA法检测各组儿童外周血IFN-γ、IL-4、IL-10及 IL-12水平,并进行比较分析。结果 RSV 感染致毛细支气管炎组外周血CD44表达及IL-4水平均显著高于RSV感染非毛细支气管炎组及健康对照组(P均〈0.01),RSV感染致毛细支气管炎组患儿外周血 IFN-γ 水平显著低于 RSV 感染非毛细支气管炎组及健康对照组(P均〈0.01)。RSV感染致毛细支气管炎组及RSV感染非毛细支气管炎组外周血IL-12水平均显著低于健康对照组(P均〈0.01)。RSV感染患儿外周血 CD44的表达水平与 IL-4水平呈正相关(r=0.687,P 〈0.01),IL-12水平与IL-4水平呈负相关(r= -0.175,P 〈0.05)。结论 黏附分子CD44在RSV感染引起的毛细支气管炎中起一定作用,高表达CD44的患儿在感染RSV后患毛细支气管炎的易感性可能增加。Objective To investigate the role of CD44 in the susceptibility to respiratory syncytial virus (RSV) associated bronchiolitis. Methods Eighty children with RSV infection [ 47 cases with post-bronchiolitis (PB) and 33 cases without post-bronchiolitis wheeze (NPB) ] who were hospitalized in Wuxi People's Hospital Affiliated to Nanjing Medical University were enrolled along, and 40 healthy children were selected as controls. The proportions of peripheral blood CD44 cells were determined by flow cytometry. The levels of cytokines (TNF-γ, IL-4, IL-10 and IL-12) in serum were determined by enzyme linked immunosorbent assay. Results The percentage of CD44 cells in peripheral blood and the IL-4 in serum in RSV associated PB group were significantly higher than those in RSV infection NPB group and control group( all P 〈0.01 ). The level of IFN-γ in RSV associated PB group was significantly lower than that in RSV infection NPB group and control group ( all P 〈 0.01 ). Moreover, the levels of CD44 was positively correlated with the amount of IL-4(r =0.687,P 〈0.01 ). The levels of IL-12 in RSV associated PB group and RSV infection NPB group were significantly lower than that in control group ( all P 〈0.01 ). The level of IL-12 was negatively associated with the level of IL4 ( r = - 0. 175,P 〈 0.05 ). Conclusions The expression of CD44 may play a role in the onset of RSV associated PB and the high expression of CD44 in children infected with RSV may increase the susceptibility to PB.
