宣肺平喘方治疗咳嗽变异性哮喘的有效性和对FeNO影响临床研究

摘　　要：目的：评价宣肺平喘方治疗咳嗽变异性哮喘的有效性和对FeNO影响。方法采用开放的、前瞻性临床研究。病例选自2011年09月-2012年10月在我院呼吸科就诊，并被诊断为咳嗽变异性哮喘的患者，入选时咳嗽症状积分至少〉12分。治疗用药为宣肺平喘方饭后口服，总疗程4周。主要疗效评价指标：咳嗽严重程度评价和FeNO的变化。结果：共入组82例，其中男性32例女性50例。年龄18～70岁平均年龄52．6岁。入选患者咳嗽时间2月～10年。纳入疗效分析的共82例，治疗后痊愈48例，占58．54％，显效29例，占35．36％，进步4例，占4．88％，无效1例，占1．22％，总有效率占93．90％。治疗后日间咳嗽评分、夜间咳嗽评分以、咳嗽总评分值及FeNO与治疗前相比差异均有统计学意义P〈0．001。纳入安全性分析共82例2例发生不良事件，发生率为2．44％，不良反应均为轻度反应，本研究中无严重不良事件发生。结论宣肺平喘方咳嗽变异型哮喘的，临床疗效显著，不良反应轻，该药可以在临床上推广应用。Objective ： To evaluate the efficacy and the influence on fractional exhaled nitric oxide （FeNO） of Chinese herbal medicine Xuanfei Pingchua Recipe on cough variant asthma（CVA） patients. Methods：An open, perspective trial was conducted. Subjects who met the diagnostic standard of CVA and with cough symptom score greater than 2 were recruited by our hospital at the department of respiration. The test Chinese herbal medicine Xuanfei Pingchua Recipe was orally administrated to subjects after meals 2 times per day for 4 weeks treatment. The primarily endpoint was the evaluation of cough severity and the change of FeNO. Results ：There were 89 subjects randomized to study treatment （FAS） ,with 32 male and 50 female. The average age for FAS was 52.6 ,with a age range of 18 ~ 70. Cough duration of patients enrolled in the study rangs from 2 months to 10 nears. Of the 82 pa tients meeting the Per protocol set （PP） ,48 patients symptoms were relieved （58.54%） ,29 patients significantly improved （35. 36% ） ,4 patients slightly better （4.88%） ,and 1 patients not improved（ 1.22 % ）. The average cough score of daytime and noc turnality,total cough score ,cough severity score and FeNO were statistically significantly lower （P 〈 0. 0001 ）. 82 patients were totally used for safety analysis,in which 2 patients （2.88%） bad minor side effects. Serious side effects were not observed during the whole study. Conclusion：Chinese herbal medicine Xuanfei Pingehua Recipe resulted in clinically important and statisti cally significant impr6ving symptoms of patients with CVA. In addition, Chinese herbal medicine Xuanfei Pingchua Recipe is asso ciated with minor or milder side effects. These results suggest that Chinese herbal medicine Xuanfei Pingchua Recipe as a safe and efficient drug can be widely used in CVA treatment.
