摘  要:目的探讨微创人工全膝关节置换术(TKA)后应用持续性被动运动(CPM)进行膝关节功能锻炼的最佳时机。方法 60例单侧TKA患者均分为三组:A组术毕即开始应用CPM,B、C组分别于术后3、5d开始应用CPM。术前、CPM后7、14d采用HSS评分评定膝关节功能。结果三组均能完成CPM锻炼计划,无一例发生感染,术后14d膝关节屈曲度均大于90度。CPM后7和14d,A组HSS评分分别为(84.55±5.5)分和(92.75±1.2)分,明显高于B组的(78.95±3.7)分和(86.70±3.0)分以及C组的(71.60±3.3)分和(85.25±4.2)分(P<0.05)。结论术后麻醉尚未清醒前即开始应用CPM更有利于改善关节功能。Objective To study the choice of time to start continuous passive motion(CPM) in the patients after total knee arthroplasty(TKA). Methods Sixty patients underwent TKA with miniincision were egually divided into three groups and CPM physical training was performed at the end of operation(group A), on the 3rd day(group B) and 5th day after surgery(group C), respectively. The knee function was evaluated using hospital for special surgery knee scores (HSS) before operation, on the 7th and 14th day after operation. Remflts The CPM physical training was completed smoothly in three groups without infectious case. The flexion of all knees was greater than 90 degrees on the 14th day after operation. The HSS scores on the 7th and 14th day in group A were (84. 55± 5.5) points and (92. 75 ± 1.2) points, respectively, which were higher than (78. 95 ±3.7) points and (86. 70±3.0) points in group B and (71. 60 ± 3.3) points and (85.25 ± 4. 2) points in group C (P〈0. 05). Conclusion Early application of CPM at the end of operation is more effective in improving knee function in patients after TKA.
