妊娠高血压疾病患者治疗前后血清白介素-4和γ-干扰素水平的变化

摘　　要：目的 探讨妊娠高血压疾病（HDCP）患者治疗前后血清白介素-4（IL-4）和γ-干扰素（IFN-γ）水平变化.方法 选择2008年1月～2013年4月在缙云县人民医院妇产科就诊治疗的HDCP患者50例为治疗组,另选择同期产前检查的正常妊娠期孕妇30例作为对照组.治疗组予以解痉、镇静、降压和利尿等常规综合治疗1周.测定治疗组患者治疗前后及对照组血清IL-4和IFN-γ水平的变化.结果 治疗组患者治疗前血清IL-4水平[（0.31±0.09）μg/L]明显低于对照组[（0.87±0.21） μg/L],血清IFN-γ水平[（48.25±9.14）μg/L]明显高于对照组[（14.69±3.12） μg/L],差异均有高度统计学意义（t=3.28、4.15,P＜ 0.01）.治疗1周后,治疗组患者血清IL-4水平[（0.59±0.12）μg/L]较治疗前[（0.31±0.09） μg/L]明显上升,血清IFN-γ水平[（29.72±5.27）μg/L]较治疗前[（48.25±9.14）μg/L]明显下降,差异有统计学意义（t=2.39、2.17,P＜0.05）.结论 HDCP患者外周血Th 1/Th2型细胞因子的比例失衡,免疫平衡由Th2细胞因子优势向Th1优势漂移,从而表现出HDCP的各种症状.治疗后在一定程度上能改善纠正外周血Th 1/Th2型细胞因子的比例失衡,使Th1/rh2平衡从Th1优势向Th2优势漂移.可见调节血清IL-4和IFN-γ水平可作为HDCP患者疗效观察和预后评估的指标.Objective To explore changes of serum Interleukin-4 （IL-4） and Interferon-γ（IFN-γ） levels of patients with hypertensive disorder complicating pregnancy （HDCP） before and after therapy.Methods 50 patients with HDCP in People＇s Hospital of Jinyun County from January 2008 to April 2013 were selected as treatment group,30 pregnant women in normal pregnancy in the same prenatal examination were selected as control group.Patients in treatment group were given routine comprehensive treatment for 1 week,such as spasmolysis,sedative,depressurization,diuresis.The changes of serum IL-4 and IFN-γlevels of patients in treatment group before and after therapy were detected.Results The serum IL-4 level of patients in treatment group before the treatment [（0.31 ±0.09） μg/L] was much lower than that in control group [（0.87±0.21） μg/L],while serum IFN-γ level [（48.25±9.14） μg/L] was much higher than that in control group [（14.69±3.12） μg/L],the differences were statistically significant （t =3.28,4.15,P 〈 0.01）.After 1 week treatment,the serum IL-4 level of patients in treatment group [（0.59±0.12） μg/L] was higher than that before the treatment [（0.31±0.09） μg/L],and serum IFN-γlevel [（29.72±5.27）μg/L] were lower than before the treatment [（48.25±9.14） μg/L],the differences were statistically significant （t =2.39,2.17,P 〈 0.05）.Conclusion The proportion of Th1/Th2 cytokine is unbalanced in peripheral blood of patients with HDCP,and immunologic balance drifts from Th2 cytokine superiority to Th1 cytokine superiority,which shows HDCP symptoms of all kinds.After medical treatment,the Th1/Th2 cytokine imbalance in peripheral blood is improved and corrected in some extents,which makes the Th1/Th2 balance drifts from Th1 cytokine superiority to Th2 cytokine superiority.Thus,the adjustment of serum IL-4 and IFN-γlevels can be the indexes of curative effect observation and prognosis evaluation of patients with HDCP.
