绿脓杆菌MSHA菌毛株菌苗对反复发作性尿路感染的治疗及其机制研究

摘　　要：采用绿脓杆菌 MSHA菌毛株菌苗 (PA - MSHA菌苗 )治疗了 40例使用抗生素无效的 RU TI,取得了满意的疗效 ,有效率 95 % (38/ 40 ) ,治愈率 92 .5 % (37/ 40 )。对其中 18例进行了治疗前后血清抗MSHA菌毛株抗体效价和外周血 T淋巴细胞亚群测定。疗后患者血清中抗 MSHA菌毛株抗体效价较疗前增高 8～ 6 4倍 ;同时外周血 T淋巴细胞亚群的比例得到了改善 ,CD+ 3 和 CD+ 4 T细胞的百分率显著提高 (P<0 .0 1) ,而 CD+ 8T细胞的百分率则显著下降 (P<0 .0 0 1)。治疗过程未见明显的副作用。研究表明 :PA-MSHA菌苗对反复发作性尿路感染治愈率高 ,安全可靠 ,患者的免疫低下状态得到改善 ,且对多种病原菌(包括革兰阴性杆菌及其 L 型、金黄色葡萄球菌及其 L 型 )所致的 RU TI效果都很好。Used the MSHA-piliated strain vaccine of Pseudomonal auroginosa (PA-MSHA vaccine) to treat 40 cases of RUTI with invalid antibiotic therapy, and gained satisfactory curative effects with effective rate 95 percent and cure rate 92.5 percent.Serum anti-MSHA piliated strain antibody and peripheral blood T lymphocyte subsets were measured in 18 of the patients.The posttherapy antibody titers increased as great as 8 to64 times that of pretherapy.After the vaccine therapy,the CD + 3 and CD + 4 percentage were significantly increased ( P <0.001),whereas the CD + 8 percentage was significantly decreased ( P <0.001).During the treatment,no side effect was found.This study confirms that the PA-MSHA vaccine:(1)it is highly curative against RUTI caused by various pathogens(G-bacilli and their L forms,Staphylococci aureus and their L forms);(2)it is safe and reliable;(3)it can improve immune state of the patients.
