内镜面罩在无痛支气管镜检查中的应用

摘　　要：目的探讨内镜面罩在无痛支气管镜检查中的安全性。方法选取12例患者,使用5#内镜面罩吸纯氧3 min后,芬太尼0.05 mg+异丙酚1～2 mg/kg诱导麻醉,面罩侧孔连接麻醉机,随自主呼吸手法辅助通气,使气道压控制在20 mm H2O(1 mm H2O=0.0098 kPa)以下,分钟通气量6～8 L。气管镜经面罩插入孔插入镜检。监测检查前、检查中、完全清醒时的血压、脉搏、呼吸、脉搏血氧饱和度,检查中的不良反应,麻醉状态对操作的影响。结果所有患者均顺利完成支气管镜的检查,未出现恶心呕吐、误吸、心律失常、低氧血症、插入困难及操作活动不便等不良反应。患者对检查均无痛苦及其他不适感觉记忆,愿意再次接受检查率为100%。结论内镜面罩在无痛支气管镜检查中能进一步提高支气管镜检查的安全性,改善患者的舒适度。Objective To investigate the safety of endoscopy mask in bronchoscopy examination.Methods Twelve patients were selected,after 3 minutes of oxygen by using 5# endoscopic mask,fentanyl 0.05 mg and propofol 1-2 mg/kg for induction of anesthesia,mask side hole connection anesthesia machine,spontaneous breathing techniques assisted ventilation,airway pressure controled under 20 mm H2O,minute volume 6-8 L.The blood pressure,pulse,respiration,SpO2 were detected before the examination,during the examination and patients were fully awake.The complication during the examination and the effect of anesthesia impact on operations were observed.Results All cases were able to complete bronchoscopic examination,and there was no nausea,vomiting,aspiration,arrhythmia,low oxygen,insert difficulties,operating activities inconvenience and other adverse reaction.The patients did not appear pain or other unpleasant sensory memory,all willing accept inspection once again.Conclusion Endoscopic mask in the painless bronchoscopy can further improve the security in the examination of bronchoscopy,and improve patient’s comfort.
