摘　　要：目的检测脓毒症患儿外周血皮质醇及白细胞介素-6(IL-6)水平的变化,评价应激剂量甲泼尼龙(MP)治疗对小儿脓毒症是否具有保护作用。方法将符合脓毒症诊断标准的患者随机分为观察组(35例)和对照组(35例)。观察组在常规治疗基础上给予小剂量MP 1～2 mg/(kg.次)治疗;对照组给予常规治疗。于治疗前及治疗后第3天检测患儿血浆皮质醇、IL-6水平。通过组内治疗前后对比及组间比较观察小剂量糖皮质激素对脓毒症患儿皮质醇、IL-6的影响。结果①治疗后观察组皮质醇水平高于治疗前,也高于治疗后对照组水平,差异均有统计学意义(P均<0.05)。②治疗后观察组IL-6水平低于治疗前,也低于治疗后对照组水平,差异均有统计学意义(P均<0.05)。结论脓毒症患儿早期可能存在绝对或相对肾上腺功能不全,小剂量MP治疗脓毒症患儿有临床意义。Objective To detect the peripheral blood cortisol and interleukin-6 levels changes in sepsis children,and evaluate the protective effect of stress-dose of methylprednisolone on sepsis children.Methods The sepsis children were randomly divided into observation group（35 cases） and control group（35 cases）.Patients in observation group were given low-dose methylprednisolone（MP） treatment and routine treatment.Patients in control group were given routine treatment.The levels of plasma cortisol,interleukin-6 were detected before treatment and 3 days after treatment.Results ①After treatment,the cortisol level in the observation group was significantly higher than before treatment and that of the control group（P＆lt;0.05）.②In the observation group,the interleukin-6 level after treatment was significantly lower than before treatment and that of the control group（P＆lt;0.05）.Conclusion The treatment of stress-dose methylprednisolone has clinical significance in patients with sepsis.
