鼻内镜手术治疗鼻息肉及慢性鼻窦炎的临床疗效观察

摘　　要：目的：探讨鼻内镜手术治疗鼻息肉及慢性鼻窦炎的临床疗效。方法回顾性分析2012年1月～2013年1月在我院行手术治疗的鼻息肉及慢性鼻窦炎患者100例的临床资料，根据治疗方法不同随机分为A组和B组各50例，A组采用鼻内镜手术：B组采用传统手术：比较A组与B组术后总有效率及术后并发症，包括术后出血、发热、鼻腔粘连、上颌窦口狭窄或闭锁。结果A组与B组术后总有效率分别为92.0%，70.0%，两组总有效率组间比较，差异具有统计学意义（x2=5.273，P＜0.05），A组无一例发生术后出血、感染，而B组均出现出血、发热、鼻腔粘连、上颌窦口狭窄或闭锁，两组并发症发生率分别为6.7%、30.0%，组间比较差异有统计学意义（x2=9.238，P＜0.05）。结论鼻内镜手术治疗鼻息肉及慢性鼻窦炎疗效确切，易于操作，具有微创、安全性好、并发症少，适合广大基层单位广泛推广和应用。Objective To investigate the clinical efficacy of endoscopic surgery in the treatment of nasal polyps and chronic sinusitis. Methods The clinical data of 100 cases with nasal polyps and chronic sinusitis underwent surgical treatment from January 2012 to January 2013 in our hospital were retrospectively analyzed. All patients were randomly divided into the group A and the group B according to the different treatment methods,each group had 50 cases,the patients in the group A were treated with endoscopic surgery,while the patients in the group B were treated with conventional surgery.The total effective rate and postoperative complications, including postoperative bleeding,fever,nasal adhesions, maxillary sinus stenosis or atresia,were compared between the group A and the group B. Results The total effective rate of the group A and the group B were 92.0%,70.0%respectively,the difference was significant in the total effective rate between the two groups（x2 =5.273,P〈0.05）;No a case in the group A happened postoperative bleeding and infection, while all patients in the group B happened bleeding,fever,nasal adhesions, maxillary sinus stenosis or atresia,the incidence of complication of the two groups were 6.7%,30.0% respectively,the difference was significant between the two groups（x2 =9.238,P 〈 0.05）. Conclusion Endoscopic surgery in the treatment of nasal polyps and chronic sinusitis has better efficacy,and is easy to operate,also has minimally invasive,good safety,fewer complications,and is suitable for the wide popularization and application of the grassroots units.
