摘 要:目的初步探讨慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者支气管肺泡灌洗液(bronchoalveolar lavage fluid,BALF)中血小板微粒(platelet microparticles,PMPs)、内皮细胞微粒(endothelial microparticles,EMPs)数目的变化情况。方法通过纤维支气管镜获取灌洗液样本,其中慢性阻塞性肺疾病急性加重期(acute exacerbations of chronic obstructive pulmonary disease,AECOPD)组35例,慢性阻塞性肺疾病(COPD)稳定期组12例,对照组20例。将所采集的样本进行离心处理后,用相应抗体进行标记,使用流式细胞技术检测每100μL灌洗液中以上2种微粒的数目。结果 PMPs数目和EMPs数目在3组之间有统计学差异(P<0.05),AECOPD组2种微粒数目均显著低于COPD稳定期组和对照组(P<0.05,P<0.01),而COPD稳定期组与对照组比较2种微粒数目均无统计学差异(P>0.05)。2种微粒数目在吸烟组与不吸烟组之间均无统计学差异(P>0.05)。性别、年龄、体表面积与BALF中2种微粒的数目变化无关(P>0.05)。结论在COPD患者中,其BALF中PMPs、EMPs数目是减少的,并且在疾病的急性加重期更明显,BALF中2种微粒数目的减少可能与疾病分期有关。Objective To explore the changes of the platelet microparticles (EMPs) in the bronchoalveolar lavage fluid of patients ease (COPD). Methods The lavage fluid specimens were obtained ding 35 cases in acute exacerbations of chronic obstructive pulmonary microparticles (PMPs) and endothelial with chronic obstructive pulmonary dis- through fiberoptic bronchoscopy, inclu- disease (AECODP) group,20 cases in normal control group (no smoking for at least 2 months) and 12 cases in stable COPD group. The specimens were centrifuged and labeled with corresponding antibodies. The numbers of the 2 types of microparticles were analyzed by flow cytometry. Results The numbers of PMPs and EMPs were significantly different in the 3 groups (P 〈 0. 05 ). Comparing with the stable COPD group and the control group, the numbers of the 2 types of nficroparticles in AECOPD group were significantly reduced (P 〈 0.05, P 〈 0.01 ). Comparing with the con- trol group, the numbers of the 2 types of microparticles in the stable COPD group had no significant difference ( P 〉 0. 05 ). There was no significant difference between the smoking group and the non-smoking group ( P 〉 0. 05 ). The numbers of the 2 types of microparticles had no correlation, with sex, age and body surface area. Conclusion The numbers of PMPs and EMPs in the bronchoalveolar lavage fluid of patients with COPD are reduced, especially in the acute exacerbation period. Reduction of PMPs and EMPs may be associated with the stages of the disease.
