摘　　要：目的对比分析CT Robson分期与TNM分期诊断肾细胞癌的价值。方法回顾性分析经手术病理证实的129例肾细胞癌(RCC)的CT图像,依据Robson分期法和TNM分期法进行术前CT分期;将两种分期结果与病理分期结果进行比较,计算两种CT分期法诊断的正确率。结果 CT Robson分期、TNM分期中的T分期和M分期与病理分期的一致性好(Kappa值=0.75、0.74、0.54)。Robson分期的准确率为81.40%(105/129),TNM总体分期的准确率为69.77%(90/129),二者差异有统计学意义(χ2=4.73,P=0.03)。结论 CT是肾细胞癌术前分期的有效方法。Robson分期的准确性高于TNM分期。Objective To compare diagnosis value of CT Robson staging and TNM staging of renal cell carcinoma （RCC）. Methods Totally 129 patients with RCC confirmed by surgical pathology were collected, and CT images were analyzed and evaluated according to Robson staging and TNM staging, and then compared with pathology staging to calculate the accuracy. Results The consistency of Robson staging, T staging and M staging of TNM to pathological staging were good（Kappa=0.75, 0.74, 0.54）. The accuracy of CT Robson was 81.40% （105/129）, while of TNM was 69.77% （90/129, χ2=4.73, P=0.03）. Conclusion CT is an effective method for preoperative staging of RCC. The accuracy of Robson staging is higher than that of TNM staging.
