摘  要:目的:探讨异丙酚联合芬太尼静脉麻醉下经阴道取卵术的镇痛效果及对体外授精一胚胎移植(IVF—ET)妊娠结局的影响。方法:回顾性分析2011年1月~12月在解放军第105医院生殖医学中心行IVF—ET治疗313个周期临床资料,镇痛方式分别采用异丙酚联合芬太尼复合麻醉(麻醉组,135个周期)和取卵前20min肌肉注射杜冷丁50mg+非那根25mg(对照组,178个周期),比较两组的镇痛效果和IVF—ET妊娠结局。结果:麻醉组患者均意识丧失,手术过程顺利;两组间患者授精率、卵裂率、优质胚胎率、临床妊娠率、早期流产率及累计妊娠率均无统计学差异(P〉0.05)。结论:异丙酚联合芬太尼静脉麻醉下行取卵术镇痛效果显著,且不影响IVF—ET临床结局,值得临床推广。Objective : To explore the clinical value of intravenous anaesthesia through combination of propofol and fentany applied in transvaginal ooeyte retrieval during in vitro fertilization -embryo transfer (IVF -ET) . Methods: From 2011 Jan to Dec in our hospital, a total of 313 cycles records of patients underwent IVF - ET were analyzed retrospectively. The 135 cycles in anaesthesia group were adminis- trated propofol and fentanyl before transvaginal oocyte retrieval performed, while 178 cycles by intramuscular injection 70 mg pethidine and 25 mg diprazine were served as a control group. The analgesia effects and clinical results between two groups were compared. Results: All the patients in the anaesthesia group were taken transvaginal oocyte retrieval successfully. The rates of fertilization, embryo cleaved, good - quality embryos, clinical pregnancy, early miscarriage and cumulative pregnancy showed no significant differences between two groups ( P 〉 O. 05 ) . Conclusion : Intravenous anaesthesia through combination of fentanyl and propofol in transvaginal oocyte retrieval could obtain satis- factory analgesia results without potential adverse effects on the outcomes of IVF - ET, and worth to populate and apply in clinic.
