无创正压通气在高龄急性左心衰竭患者治疗中的应用

摘　　要：目的探讨无创正压通气(NIPSV)在高龄急性左心衰患者治疗中的临床应用价值。方法收集40例高龄急性左心衰病人,分成无创正压通气组及常规治疗组,无创正压通气组采用常规药物治疗+无创正压通气,常规治疗组采用常规药物治疗,对两组治疗前、治疗2h和24h后症状、肺部啰音、血气及两组治疗前、治疗24h后超声心功能、BNP等指标进行对照分析。结果无创正压通气组在短时间内各项指标得到改善,明显优于常规治疗组。结论应用NIPPV治疗急性左心衰能快速改善氧合及呼吸困难,并改善心功能,值得在临床推广使用。Objective To explore the clinical application of non--invasive positive pressure ventilation （NIPPV） in the treatment of the elderly patients with acute left heart failure. Methods 40 cases of the elderly patients with acute left heart failure were divided into groups of non--invasive positive pressure ventilation and routine treatment group, the former adopted routine drug treatment and non--invasive pos- itive pressure ventilation, and the latter were given conventional drug treatment. Of the two groups, con- trast analysis the indicators such as symptoms, lung tale and blood gas 2 hours before treatment and 24 hours after treatment. At the same time, contrast analysis was made of such indicators as ultrasonic cardiac function and the BNP before treatment and 24h after treatment. Results All indicators of the treat- ment group are improved in a short time. The treatment group is superior to the routine drug treatment group. Conclusion Compared with routine treatment, the application of NIPPV treatment of acute left heart failure can quickly improve oxygenation and breathing difficulties, and improve heart function which is worth popularizing in clinical use.
