区域维护模式在留置PICC患者中的应用

摘　　要：目的:探讨PICC区域维护模式在留置PICC患者中的应用与效果。方法:设立PICC区域维护点,对区域维护成员进行PICC培训,建立PICC区域维护点及成员联系表,建立留置PICC患者置管维护档案,对留置PICC患者及家属实施安全和健康教育。结果 :118例留置PICC患者在区域维护点接受维护,仅5例(4.2%)发生了并发症,其中堵管1例,穿刺点感染1例,导管部分脱出1例,皮肤过敏1例,置管肢体肿胀1例;97.5%的患者对区域维护模式及区域维护点的服务态度、维护技术和收费感到满意。结论:PICC区域维护模式的建立,解决了PICC带管患者的后顾之忧,是一种安全可行并达到了护患双赢目的的好方法,为较理想的院外护理模式。Objective： To discuss the application and effect of the Regional Nursing Maintenance Program （RNMP） in community hospital for patients discharged with indwelling PICC catheter. Methods： The authors selected and set up PICC regional maintenance centers （RMC） in community hospitals according to patients needs. They prepared contact information list and nursing care files for them, and provide training to the RMC members and education to the patients and their family members. Results： A total of 118 patients discharged with indwelling PICC catheter received nursing care in PICC RMCs. Only 5 cases （4.2%） had complications, including occlusion （1 case）, infection （1 case）, malposition （1 case）, skin irritation （1 case）, and limb swelling （1 case）. 97.5% of the patients were satisfied with the service, care skills and charge issues. Conclusion： The establishment of PICC RMC could meet the needs of community patients discharged with indwelling PICC catheter, and it is a safe way to achieve win-win goal as an ideal model of community care for patients discharged from tertiary hospital.
