摘　　要：【目的】　探索建立更加适合评价新生儿体格发育的新身体指数。　【方法】　通过 13 4 1例足月适于胎龄 (AGA)儿的身长、体重、头围用计算机建立新的身体指数—张路指数 :体重 (g) +2 5 0头围 (cm)×身长 (cm)× 2 (简称ZLI) ,并通过 14 4例小于胎龄 (SGA)儿、167例大于胎龄 (LGA)儿及 75例足月AGA儿的原始数据对新指数及其它 10项身体指数与贯用的胎龄—体重法进行反馈性验证比较。　【结果】　ZLI随胎龄的增加而增大 ,其中胎龄 3 1～ 3 3周和 3 4～ 3 5周ZLI各组间差异有显著意义 (P <0 .0 5 ) ;ZLI随生后日龄的增大而增加 ,各时段间差异有显著意义 (P <0 .0 1) ;ZLI和其它 10项身体指数与胎龄—体重法比较 ,其灵敏度、特异度和总符合率分别为 70 %、75 %和 73 % ,漏诊率和误诊率分别为 3 0 %和 2 5 %。与其它 10项指数比较 ,正常值波动范围小 ,为 1.0 0± 0 .0 6( x±s) ,均值为 1.0 0容易记忆 ,包含的参量多 (含有新生儿体格发育最重要的三项指标 ) ,更符合综合评价的要求。　【结论】　新建立的ZLI是一个综合评价新生儿宫内及生后 0～ 2月小儿体格发育的好指数。Objective To establish a new index for comprehensive evaluation of physical growth of neonates. Methods Zhang Lu Index(ZLI):Weight(g)+250height(cm)×head circumference(cm)×2 was computerized with data of body height, body weight and head circumference collected from 1 341 full term AGA neonates. This index as well as other 10 boby indices were compared to the conventional body weight for gestational age based on data coming from 144 SGA, 167 LGA and 75 AGA neonates. Results Differences among ～31,～32,～33 gestational weeks and between ～34 and ～35 gestational weeks were significant respectively(P<0.05). After birth,ZLI increases along with the daily age. The differences among the daily age in ～3days,～7days,～14days,～28days,and ～60days were also significant (P<0.01). Compared to the method of body weight for gestational age, and other 10 body indices the sensitivity, specificity,loss diagnositic rate,false diagnositic rate as well as the total compatible rate for ZLI were 70%,75%,30%,25% and 73% respectively. The normal values were fluctuated in small range (±s,1.00±0.06) which included more parameters (height,weight and head circumference) were easy remembered. Conclusions ZLI is a better indicator for comprehensively evaluating the growth of intrauterine and in postnatal 2 months of life.
