摘  要:目的应用扩散张量成像(DTI)技术对肌萎缩侧索硬化症(ALS)的各向异性分数(FA)进行对照分析,为ALS的早期诊断提供上运动神经元损害的客观依据。方法病例组56例,对照组(健康志愿者)23名,行常规MRI及轴位DTI,选取感兴趣区测量各向异性分数(FA)值,使用GE FunctoolⅡ软件处理DTI图像,测量FA值。结果与对照组比较,病例组大脑脚、内囊后肢FA值降低,中央前回FA值差异无统计学意义。当大脑脚取截断点FA≤0.686时,SE为95.7%,1-SP为16.1%,受试者工作特征曲线下面积为0.937,95%的可信区间为0.885~0.989,标准误为0.027。当内囊后肢取截断点FA≤0.708时,SE为95.7%,1-SP为14.3%;受试者工作特征曲线下面积为0.934,95%的可信区间为0.882~0.987,标准误为0.027。结论 MR DTI技术对ALS的早期诊断有重要价值。Objective To analyze the value of diffusion tensor imaging (DTI)in the diagnosis of amyotrophic lateral sclerosis (ALS). Methods 56 patients with ALS and 23 Healthy volunteers underwent MRI and DTI examination. FA values were measured using functool H software of GE. Results ( 1 ) Compared with Control group, FA of Cerebral pe duncle and posterior limb of internal capsule decreased in ALS, while there is no statistical significance in subcortical white matter( SWM ) of precentral gyrnl between two groups ; (2) When FA value of Cerebral peduncle was equal to 0. 686 or low er, SE was 95.7% , 1SP was 16. 1% , ROC curve area was 0. 937, 95 percent confidence interval was from 0. 885 0. 989, standard error was 0. 027. When FA value of posterior limb of internal capsule was 0. 708 or lower , the SE was 95.7% , 1SP was 14.3% , ROC curve area was 0.934, 95 percent confidence interval was from 0. 882 0. 987, standard error was 0.027. Conclusion Magnetic resonance DTI technology can provide useful information in diagnosing and fur ther understanding ALS.
