彩色多普勒超声对糖代谢异常患者视网膜中央动脉血流动力学的研究

摘　　要：目的探讨彩色多普勒超声对糖代谢异常患者视网膜中央动脉(central retinal artery,CRA)早期病变的诊断价值。方法根据患者空腹血糖及口服葡萄糖耐量试验血糖值结果分为4组:A组(血糖正常组)25例,B组(新增空腹血糖受损组)22例,C组(糖尿病前期组)22例,D组(2型糖尿病组)31例。对各组患者均行视网膜中央动脉彩超检测,获得血流频谱,并测量收缩期峰值流速(PSV),舒张末期流速(EDV),平均流速(Mn),阻力指数(RI),收缩期峰值流速与舒张末期流速比值(S/D)等参数。结果与A组比较:D组CRA的PSV、EDV、Mn显著下降,RI、S/D显著升高,均有统计学意义(P<0.05);C组CRA的PSV下降显著,有统计学意义(P<0.05);B组CRA的各项血流参数虽有变化趋势,但差异不显著(P>0.05)。结论糖尿病前期已经发生了CRA的血流动力学改变,PSV是比较敏感的指标。彩色多普勒超声动态监测糖代谢异常人群的微血管血流动力学状态,对早期诊断、干预视网膜病变进展有重要意义。Objective To investigate the value in early diagnosis by Color Doppler flowing Imaging for abnormal glucose metabolism in patients with abnormal central retinal artery （CRA）.Methods Four groups were divided according to the results of patients with fasting plasma glucose and oral glucose tolerance test.group A （normal plasma glucose group） is 25 cases ;Group B （new impaired fasting glucose group）is 22 cases; Group C （pre-diabetes group）is 22 cases; Group D （diabetes group） is 31 cases.The patients of central retinal artery were examined by pulse Doppler to get flow spectrum,then measured the parameters of the peak systolic velocity （PSV）,end diastolic velocity （EDV）,mean velocity （Mn）,resistance index （RI）,systolic peak velocity and end-diastolic velocity ratio （S/D）.Results With the glucose metabolic abnormalities,compared with group A,PSV,EDV,Mn in group D decreased significantly,RI and S/D rised significantly （P〈0.05）.The PSV of CRA in group C was higher than in group A（P〈0.05）; There was no statistical significance in the parameters of CRA between group B and group A（P〉0.05）.Conclusions Central retinal artery hemodynamics have changed in abnormal glucose metabolism patients,PSV is a sensitive index.
