摘  要:目的探讨伴有明显声影甲状腺微小癌的超声诊断。方法对33例经手术病理证实,伴有明显声影≤1 cm甲状腺单发实性结节进行回顾性研究。根据病理结果,将其分为良性组(n=17)和恶性组(n=16),对其超声特征包括形态、回声水平和微钙化等进行分析。明显声影定义为结节内强回声结构后方宽大的无回声区,完全覆盖结节后方,使结节后面部分结构完全无法显示。结果 2组中结节形态、微钙化和回声水平比较差异均具有统计学意义(P<0.05)。以单一结节形态不规则、微钙化和低回声预测甲状腺微小癌,准确性分别为85%、79%和70%。结合以上三者预测甲状腺微小癌,阳性预测值为100%。结论形态不规则、微钙化和低回声有助于预测伴有明显声影的甲状腺微小癌。Objective To investigate the value of ultrasonography for diagnosis of thyroid microcarcinoma with obvious posterior acoustic shadow.Methods Thirty-three solitary solid thyroid nodules less than or equal to one centimeter with obvious posterior acoustic shadow were reviewed retrospectively.On the basis of histopathological results,the nodules were divided into two groups:benign group (n=17) and malignant group (n=16).Ultrasonographic features of nodules including shape,echogenicity and microcalcification were analyzed.Obvious acoustic shadow is defined as broaden anechoic areas posterior to hyperechonic structure,the posterior part of the nodule could not be detected by ultrasonography.Results Significant difference between the two groups in shape,microcalcification and echogenicity of nodules was observed (P〈0.05).With single ultrasonographic feature such as irregular shape,or microcalcification or hypoechoicity for predictive microcarcinoma,the accuracy were 85 %,79 % and 70 %,respectively.With combination with above mentioned three ultrasonographic features to predictive microcarcinoma,the positive predictive value was 100%.Conclusions Irregular shape,microcalcification and hypoechoicity of nodule were useful for predictive microcarcinoma with obvious posterior acoustic shadow.
