摘  要:目的评价声触诊组织定量技术(VTQ)在移植肾急性排斥(AR)诊断中的临床价值。方法选择48例移植肾功能稳定患者和经病理证实AR患者27例,对移植肾行彩色多普勒超声和VTQ检查,并对检查结果进行对比分析。结果观察者内SWV测值在移植肾中段的重复性较好(ICC>0.75),AR组SWV高于移植肾功能稳定组(P<0.05),由ROC曲线可知,当SWV截值=2.43 m/s时,其预测移植肾急性排斥的敏感性和特异性分别为92.6%和75%。结论 VTQ技术具有无创、简便及重复性高的特点,有助于移植肾急性排斥的诊断。Objective To evaluate the diagnostic value of virtual touch tissue quantification (VTQ) in transplanted kidney with acute rejection (AR). Methods 48 kidney recipients with stable renal function and 27 with acute rejection were examined by conventional ultrasound and VTQ after transplantation. Biopsies were performed in 27 patients. All the results were compared between two groups. Results Mean SWV values measured in mid part of the renal cortex were with high reproducibility (ICC〉0. 75) . The SWV values of patients with stable graft function were lower than patients with AR. According to the area under the ROC curve , The sensitivity and specificity of SWV (area under ROC curve=0. 835, cut--off=2.43 m/s ) in predicting AR was 92.6% and 75% respectively. Conclusions VTQ technique was stable for the assessment of transplanted kidney. It could provide a new method in the diagnosis of acute rejection.
