摘 要:目的探讨右美托咪定对大鼠肾缺血/再灌注损伤的保护作用。方法选择SPF级雄性SD大鼠40只,随机分为4组,每组10只,分别为对照组(S组)、缺血再灌注组(I/R组)、右美托咪定预处理组(Pre/Dex组)及右美托咪定后处理组(Post/Dex组)。S组行右肾切除;I/R组行右肾切除,左肾缺血45 min,再灌注60 min,造肾缺血再灌注模型;Pre/Dex组于大鼠股静脉穿刺置管后泵注1μg/kg右美托咪定;Post/Dex组于左肾再灌注后静脉泵注1μg/kg右美托咪定,10 min后改为0.5μg/kg,泵注30 min停止。于再灌注后6 h留取全血标本,分离血清,测定肌酐和尿素氮浓度;ELISA法检测血清IL-1β和TNF-α含量;同时,取肾组织,制作病理切片,HE染色观察各组病理组织学变化。结果肾脏病理组织学观察,S组肾脏未见明显变化,I/R组肾脏损伤严重,炎症明显,肾小管扩张,有肾小球肾炎表现;炎症因子及血生化检测结果显示,与S组相比,I/R组、Pre/Dex组和Post/Dex组血清中肌酐、尿素氮、IL-1β和TNF-α含量均显著升高(P<0.05),Pre/Dex组和Post/Dex组明显低于I/R组(P<0.05),但Pre/Dex组和Post/Dex组比较,差异无统计学意义(P>0.05)。结论右美托咪定可减轻肾缺血再灌注中的炎性反应,改善肾功能,具有一定的肾保护作用。Objective To observe the protective effect of in rat with renal ischemia reperfu- sion injury. Methods SPF male SD rats ( n = 40 ) were randomly divided into four groups : sham group ( group S ) , is- chemic reperfusion group (group I/R) , dexmedetomidine preconditioning group (group Pre/Dex) and dexmedetomi- dine post-treatment group (group Post/Dex) , 10 rats in each group. Group S was underwent right nephrectomy. An is- chemia-reperfusion model with group I/R by cutting off the right kidney, and lefting the right one ischemia for 45 min, then transfusing blood for 60 rain was built. As for group Pre/Dex, a puncatre in the rats' femoral vein, and transfused 1 μg/kg dexmedetomidine for 10 min, followed by infusing o. 5 μg/kg dexmedetomidine until SMI was made. The study reperfused in the left renal of group Post/Dex, and then infused dexmedetomidine for 30 min. Blood sample was collected in 6 h to detect the density of creatinine and urea;ELISA was used to detect the change of content of IL-1 β, TNF-α;Histopathological changes of kidney were observed by using pathological section and HE staining. Results Histopathological obseryations of kidney showed no obvious change in kidneys of group S ; Serious kidney injury was found in group I/R, which showed up as obvious inflammation, tubular dilatation, congestion, edema, glomerulonephri- tis;While the kidney injury was significantly alleviated in group Pre/Dex and group Post./Dex,and no serious inflam- matory response was detected, and unusual cellular morphology was less. Compared to group S,levels of serum creati- nine, urea nitrogen, IL-1 β and TNF-α in group I/R, group Pre/Dex and group Post/Dex had increased significantly ( P 〈 0. 05 ) , which had significantly decreased in group Pre/Dex and group Post/Dex compared to group I/R, while there was no significant difference between group Pre/Dex and group Pos/Dex ( P 〉 0. 05 ).
