间歇性束支阻滞的动态心电图诊断

摘　　要：目的观察12导联动态心电图间歇性束支阻滞病例的临床特点。方法选择2009年6月至2012年6月我院的3660例患者的12导联动态心电图，检出间歇性完全性左、右束支阻滞10例，占2．7‰，并对完全性左、右束支阻滞时QRS波时间及心率改变进行分析。结果完全性右束支阻滞7例，完全性左束支阻滞2例，左前分支阻滞1例，发现间歇性左、右束支阻滞QRS时间由〈0．12秒到20．16秒，3相、4相左束支或右束支阻滞均可出现，以3相左束支或右束支阻滞多见。结论间歇性束支阻滞发生率较低，心电图发作多呈频率依赖性或非频率依赖性，间歇性右柬支阻滞较间歇性左束支阻滞多见。Objective To observe the clinical characteristics of 12 lead dynamic electrocardiogram intermittent bundle branch block patients. Methods 12 lead dynamic electrocardiogram in 3660 patients from 2009 June to 2012 June in our hospital were detected, intermittent complete left, right bundle branch block in 10 cases, accounting for 2.7 per thousand, and the complete left, fight bundle branch block of QRS wave and heart rate change analysis. Results of the 7 patients with complete right bundle branch block, 2 cases of complete left bundle branch block in 1 cases, left anterior hemiblock, found that intermittent left, right bundle branch block in QRS time by 〈 0.12 seconds to over 0.16 seconds, the 3 phase, the 4 phase of left bundle branch or fight bundle branch block may appear, tothe 3 phase left bundle branch or right bundle branch block. Conclusion intermittent bundle branch block with low incidence, ECG episodes are frequency dependent or non frequency dependent, intermittent fight bundle branch block with intermittent left bundle branch block.
