补中益气丸配合CRFA治疗原发性肝癌临床观察

摘　　要：目的:探讨补中益气丸配合冷循环射频消融(CRFA)治疗原发性肝癌的疗效。方法:对30例患者45个肿块,在超声引导下,采用单针和集束针进行CRFA治疗。术后口服补中益气丸,每次8粒。每日3次。结果:64例患者307个肿块行368次CRFA治疗。平均每个病灶1.2次。术后1个月病灶完全坏死89.3%,部分坏死10.7%。CRFA术后83.5%患者AFP下降至正常。术后1 a生存率为82.1%,2 a生存率为63.2%,3 a生存率为44.4%。术后1个月完全坏死的病灶超声显示回声增强,部分病灶内呈囊性变。完全坏死的病灶彩色多普勒检查无血供,不完全坏死区血供减少。完全坏死病灶增强CT不强化,部分坏死者在未坏死区强化。结论:CRFA是一种安全有效治疗肝细胞肝癌的方法,其疗效主要取决于针型的选择、合理的布针以及消融的范围,补中益气丸配合CRFA治疗原发性肝癌疗效显著。Objective：To investigate the effect of Buzhong Yiqi Wan combined with cool-tip radiofrequency ablation （CRFA） in the treatment of primary hepatocellular carcinoma. Methods ：30 cases of patients with 45 tumors using single needle and cluster needle for CRFA treatment. Postoperative oral administration of Buzhong Yiqi Wan, eight pills every time,3 times a day. Results：307 tumors in 64 patients were carried out 368 times CRFA treatment, and 1.2 times average lesion. Fully necrosis lesions was 89.3 % one month after the operation, and partial necrosis was 10.7%. CRFA postoperative AFP of 83.5% of patients droped to normal. Postoperative one year survival rate was 82.1% ,two years survival rate was 63.2%, three years survival rate was 44.4%. Fully necrosis lesions ultrasound showed that echo enhancement, partly lesion with cystic change. Completely necrotic lesions had no blood supply by color doppler exam- ination, and not completely necrotic areas reduced blood supply. Fully necrotic lesions enhanced CT didn＇t strengthen,and partially nec- rosis enhanced in area of non-necrosis. Conclusion：CRFA is a safe and effective method for treatment of hepatocellular carcinoma, and its effect mainly depends on the needle selection, reasonable layout of needle and the ablation range, Buzhong Yiqi Wan combined with CRFA in treating primary hepatic carcinoma has significant curative effect.
