ENA抗体与体液免疫检验在SLE诊断中的应用价值

摘　　要：探讨EAN抗体、抗ds-DNA与体液免疫检测在系统性红斑狼疮(SLE)诊断中的应用.选取58例诊断明确的SLE患者为SLE组,另取健康人群21例为对照组.清晨空腹采静脉血,用免疫斑点法检测EAN抗体及抗ds-DNA,用散射比浊法在IMMAGE特种蛋白分析仪检测IgG、IgA、IgM、C3、C4.结果显示:SLE患者血清中IgG、IgA水平分别为(21.61±3.79)g/L、(2.96±0.52)g/L,明显高于对照组;C3、C4水平分别为(0.66±0.23)g/L、(0.16±0.05)g/L,明显低于对照组.SLE患者组中IgG、IgA、C3及C4阳性率分别为67.2%、51.7%、69.0%、65.5%,对照组阳性率低于5.0%.SLE患者血清中多种自身抗体呈阳性,抗ds-DNA、抗Sm阳性率分别为37.9%、32.8%,但是对照组中未检出抗ds-DNA、抗Sm、抗nRNP、抗SSA、抗SSB、抗ScL-70、抗Jo-1.结果表明:联合检测ENA抗体、抗ds-DNA和体液免疫有助于指导临床疾病诊断及疗效评估.To study the value of detection of extractable nuclear antigen antibodies （ENA）,anti-ds-DNA and humoral immunity in diagnosis of systemic lupus erythematosus （SLE） ,58 cases of diagnosed SLE pa- tients were chosen in SLE group,21 healthy people were chosen in control group. Fasting venous blood were collected, ENA antibodies and anti-ds-DNA were detected with dot immunobinding assay, the level of IgG, IgA, IgM, C3 and C4 were detected with turbidimetric method on IMMAGE special protein analyzer. The results showed that the level of IgG and IgA in serum of SLE patients were （21.61 ±3.79） g/L and （2.96 ± 0.52） g/L,they were significantly higher than that in control group. The level of C3 and C4 in se- rum of SLE patients were （0.66±0.23）g/L and （0.16±0.05） g/L,they were significantly lower than that in control group. The positive rates of IgG,IgA,C3 and C4 in SLE group were 67.2%,51.7% ,69.0% and 65.5%,but the positive rate in control group were less than 5%. Many kinds of auto-antibodies showed positive reaction in serum of SLE patients, the positive rate of anti-ds-DNA and anti-Sm were 37.9 % and 32.8% ,but the anti-ds-DNA, anti-Sm, anti-nRNP, anti-SSA, anti-SSB, anti-ScL-70 and anti-Jo-1 were not detected in control group. The results suggested that combined detection of ENA antibodies,anti-ds-DNA and humoral immunity can help us to guide clinical diagnosis and curative effect evaluation.
