地佐辛复合丙泊酚用于无痛肠镜诊治100例

摘　　要：目的 观察地佐辛复合丙泊酚用于无痛肠镜检查麻醉效果的有效性、安全性及可行性.方法 选择拟进行肠镜检查的患者200例,美国麻醉医师协会（ASA）分级Ⅰ～Ⅱ级,随机分为两组,各100例,分别为丙泊酚组（Ⅰ组）和地佐辛复合丙泊酚组（Ⅱ组）.观察两组患者麻醉药物起效时间、清醒时间、肠镜检查时间和丙泊酚用量,记录所有患者检查前、中、后的血压（BP）、心率（HR）、指脉血氧饱和度（Sp02）、相对危险度值及不良反应发生的例数.结果 Ⅰ组与Ⅱ组相比,两组术中与术前BP,SpO2的差值比较,差异有统计学意义（P＜0.05）,Ⅰ组有2例出现收缩压（SBP）＜ 80 mmHg,16例患者出现SpO2低于95％,Ⅱ组发生例数低于Ⅰ组（P＜0.05）,两组患者麻醉药物起效时间、肠镜检查时间比较,差异无统计学意义（P＞0.05）,Ⅱ组术毕清醒时间和全程丙泊酚用量均少于Ⅰ组（P＜0.05）,术中呼吸暂停和术后恶心呕吐发生率Ⅰ组明显高于Ⅱ组（P＜0.05）,苏醒期兴奋躁动发生率Ⅱ组明显低于Ⅰ组（P＜0.05）.结论 地佐辛联合丙泊酚麻醉效果好,安全性高,是一种安全、有效、可行的无痛肠镜麻醉方法.Objective To observe the effectiveness, safety and feasibility of dezocine combined with propofol in painless enteroscope. Methods Two hundred patients undergoing enteroseopy （ASA I-U） were selected and randomly divided into the propofol group（group Ⅰ ） and the dezocine combined propofol group （group Ⅱ ）, 100 cases in each group. The onset time of anesthetic drugs, recovery time, en- teroscopy time and propofol amount were observed and the blood pressure（BP）, heart rate（HR）, blood oxygen saturation of artery（SpOt）, relative risk（RR） value and adverse reactions before, during and after enteroscopic examination were recorded. Results The difference values of preoperative and intraoperative BP and SpO2 had statistical differences between the two groups（ P〈 0.05）. Two cases in the group I had systolic blood pressure（SBP）〈 80 mmHg and 16 cases had SpO2〈 95%, while which in the group lI were lower than those in the group I （P 〈 0. 05）. The onset time of anesthetic drugs and the enteroscopy time had no statistieal significance between the two groups（ P 〉 0. 05）. The postoperative recovery time and the propofol amount in the group I] were less than those in the group I （P 〈 0. 05）. The occurrence rates of intraoperative apnea and the postoperative nausea and vomiting in the group I was significant higher than those in the group Ⅱ（ P〈0. 05）. The occurrence rate of excitement and restless during the recovery period in the group Ⅱ was significant lower than that in the group I（ P 〈 0. 05）. Conclusion Dezocine combined with propofol anesthesia has good effect and high safety, which is a safe, effective and feasible anesthesia method for conducting painless enteroscopy.
