老年病房暴发疥疮感染的流行病学调查与控制对策

摘　　要：目的通过老年病房暴发疥疮感染的流行病学调查和总结控制对策,为有效预防和控制专科医院封闭式集中管理的老年病房皮肤感染提供依据和措施。方法于2012年2月一例自外院转来的老年患者住院一周后出现皮肤感染症状,经皮肤科会诊后,诊断疥疮,于3月先后出现多例患者及护工发生疥疮感染,对该次暴发疥疮感染事件进行流行病学调查并制定控制对策。结果感染患者17例、护工3人,皮肤检出金黄色葡萄球菌1例,其余未检出致病菌;经治疗2周后,17例患者症状控制,皮疹消褪,5例合并湿疹患者4周后完全恢复,无新发患者。结论该次疥疮感染为医院感染暴发事件,护工是主要传播者,护工手感染是造成该次暴发流行的主要传播途径,及时治疗和有效的隔离是控制疥疮流行的有效措施。OBJECTIVE To provide the basis and measures for effective prevention and control of skin infection in closed centralized geriatric ward in specialist hospitals through the epidemiological survey of outbreaks of scabies infection and the summarization of control counter measures. METHODS In Feb, 2012, skin infection symptoms occurred in one elderly patients transferred from outside of the hospital after hospitalized for one week and was di- agnosed of scabies by the dermatology consultation, then scabies occurred in many patients and care workers in March, the epidemiological investigation was performed, and countermeasures were developed for the control of outbreaks of scabies. RESULTS A total of 17 patients and 3 care workers were infected. Staphylococcus aureus was detected in skin of one patient, and no pathogens were detected in the remaining patients. Two weeks after the treatment, the symptoms were controlled in 17 patients, and the rash faded, 5 patients with eczema fully restored 4 weeks after the treatment, and no new cases were found. CONCLUSION This scabies infection is an event of hospital infection outbreak, and the care workers are the main communicators, the hand infections in the nursing staff are the main route of transmission. Timely treatment and effective isolation are the effective measures to control the prevalence of scabies.
