带血管蒂自体脾叶移植术6例报告

摘　　要：目的　介绍 6例带血管蒂自体脾叶移植术 ,探讨其优点及适应证。方法　根据脾血管进入脾门时分支为脾叶血管的解剖学特点 ,切除的脾脏体外清创后 ,行带血管蒂自体脾叶移植 ,脾叶血管和胃网膜血管吻合。结果　本组 6例术后行外周血象、各类Ig、C3、C4 、彩超及CT检查 ,并随访 6个月至 8年 ,观察到移植脾叶成活并发挥了脾功能作用 ,未出现并发症。结论　带血管蒂自体脾叶移植术 ,尽管技术难度较高 ,但作为保脾手术之一 。Objective To introduce 6 cases of transplantation of auto-splenic lobe with vessel pedicle and discuss its advantages and indications.Methods According to the anatomical characteristics that the vessels of the spleen are branched(into) vessel of splenic lobe at the phylum of the spleen,after having cleaned up the isolated spleen, we performed transplantation of auto-splenic lobe with vessel pedicle, the splenic lobe vessels being anastomosed with the gastro-mesenteric vessels.Results All of these 6 cases have undergone the examination of peripheral blood smear, the examination of the content of various Ig、C 3、C 4 in the blood, colored ultrasonography and CT. All patients have been followed up for 6 months to 8 years and we observed that the auto transplanted-splenic lobe had been alive with normal function but without any complications.Conclusion Though there is difficulty in the technical aspect of the transplantation of auto-splenic lobe with vessel pedicle, it will be of clinical value as one of spleen protection surgery.
