超声引导下美蓝定位后美容切口切除触诊阴性乳腺病灶的临床应用

摘　　要：目的：探讨超声引导下美蓝定位后采用美容切口切除临床触诊阴性的乳腺病灶的应用价值和可行性。方法：选择2009年10月-2011年10月的109例女性患者，对130枚临床触诊阴性的乳腺病灶进行超声引导下美蓝染色定位，手术时采用美容切口。结果：130枚乳腺病灶全部成功切除，其中2枚为早期浸润型导管癌，2枚为导管原位癌，1枚为小叶原位癌，3枚为重度非典型增生，乳腺癌及癌前病变检测率为7．34％，余122枚为良性，其中乳腺纤维腺瘤63枚，脂肪瘤3枚，乳腺腺病瘤18枚，积乳囊肿2枚，硬化性腺病9枚，错构瘤2枚，乳腺增生伴局部纤维腺瘤变16枚，导管内乳头状瘤3枚，乳腺轻一中度非典型增生6枚。结论：超声引导下美蓝定位后美容切口切除触诊阴性病灶的技术具有操作简便、定位准确、美容效果好等诸多优点，值得基层医院临床推广。Objective： To explore the application value and feasibility of ultrasound - guided excision of non - palpable breast le- sions after methylene blue staining with cosmetic incision. Methods： A total of 109 female patients were selected from October 2009 to Octo- ber 2011, 130 specimens of non - palpable breast lesions underwent ultrasound - guided excision after methylene blue staining with cosmetic incision. Results： All the 130 specimens of non -palpable breast lesions were excised successfully, including two specimens of early inva- sire ductal carcinoma, two specimens of ductal carcinoma in situ, one specimen of lobular carcinoma in situ, and three specimens of severe atypical hyperplasia, the detection rate of breast cancer and breast precancerous lesion was 7. 34% ; the other 122 specimens were benign, including 63 specimens of breast fibroadenoma, 3 specimens of lipomyoma, 18 specimens of breast adenosis tumor, 2 specimens of cyst of galactostasia, 9 specimens of sclerosing adenosis, 2 specimens of hamartoma, 16 specimens of breast hyperplasia and local fibroadenoma, 3 specimens of intraductal papilloma, and 6 specimens of breast mild - moderate atypical hyperplasia. Conclusion： Ultrasound - guided excision of non - palpable breast lesions after methylene blue staining with cosmetic incision has the advantages of simple operation, accurate po- sitioning, and good cosmetic effect, which is worthy to be popularized in basic hospitals.
