戒烟门诊与普通门诊戒烟效果的比较

摘　　要：目的：探讨戒烟门诊药物治疗＋专业心理、行为干预的戒烟效果。方法：将2010年1月到2010年6月收治的戒烟患者109例分为普通门诊组58例、戒烟门诊组51例，均给予伐尼克兰治疗期3个月．普通门诊组只在首次就诊时进行3min的烟草危害知识宣教．戒烟门诊组每次随访时进行20～30rain面对面的心理及行为的干预。结果：戒烟门诊组受试者9～12周、9—18周及9～24周持续戒断率明显高于普通门诊组（P〈0．05）。戒烟门诊组受试者9、12、18、24周时点戒断率明显高于普通门诊组（P〈0．05）。戒烟门诊组受试者在12、18、24周复吸率明显低于普通门诊组（P〈0．05）。戒烟门诊组受试者出现焦虑、烦躁、易激惹和体质量增加的人数明显少于普通门诊组（P〈0．05）。结论：戒烟药物伐尼克兰具有较好的有效性和安全性，药物治疗＋专业心理、行为干预是一种行之有效、可行性强的戒烟模式。Objective： To investigate the smoking cessation outcomes of combined pharmacological therapy plus specialized psychological and behavioral intervention in smoking cessation clinics. Methods： One hundred and nine patients visited the outpatient department between January 2010 and June 2010 were assigned to receive, on the basis of 3-month varenicline therapy, either a 3-minute education program of potential hazards of smoking at initial visit only （ general clinic group, n = 58 ） or a 20 - 30-minute face-to-face psychological and behavioral intervention by a specialist at each visit （ general clinic, n = 51 ）. Results ： Smoking cessation clinic gave a substantially higher continuous cessation rate during weeks 9 - 12, 9 - 18 and 9 - 24 （ all P 〈 0.05 ） and point cessation rate at weeks 9, 12, 18 and 24 compared with the general clinic （all P 〈0.05） yet reduced rate of relapse of smoking at weeks 12, 18 and 24 （all P 〈 0.05 ）. There was a reduced number of patients with anxiety, restlessness, irritability and weight gain in smoking cessation clinics （ all P 〈 0.05 ）. Conclusion ： Apart from varenicline, a stop-smoking medication, the pharmacotherapy in combination with psychological and behavioral intervention is an effective and feasible approach for smoking cessation.
