摘　　要：目的　改良带锁髓内钉治疗股骨干骨折的手术技巧。　方法　设计特制的逆行钻钉 ,经皮插入近端髓腔 ,逆行钻通梨状窝 ,至臀部穿出 ,套入特制软组织套管 ,用特制扩孔器扩孔 ,顺行插入带锁髓内钉。　结果　临床使用 5 8例 ,臀部切口由原来 8～ 10cm减至 3～ 4cm ,钻孔定位准确 ,操作简便省时。　结论　改变了现行顺行定位钻孔方法 ,避免了因此造成的各种并发症 ,损伤少 ,值得推广使用。Objective To modify the surgical technique of interlocking intramedullary nail for femoral fracture. Methods percutaneously inserting the specially designed retrograde drill into the proximal medulla,drill out the piriform recess retrogradely and the drill come out from the natis skin.Insert the specially designed soft tissue cannula into the drill and then expand the foramen with a specially designed expander.Finally insert the intralocking intramedullary nail anterogradely. Results In 58 cases,the natis incision is lessened from 8-10 cm to 3-4 cm.Drill-positioning was correct and time-saving. Conclusion The currently used anterograde drill-positioning was changed.Therefore various complications due to this were avoided.So the retrograde method is less traumatie and worth of recommending.
