摘  要:目的:采用Meta分析探讨双源CT双能量技术在孤立性肺结节(SPN)研究中的临床应用价值。方法:以Dual source CT,solitary pulmonary nodule、双源CT、孤立性肺结节等为关键词,检索2006年1月至2012年8月Cochrane图书馆、PubMed、Embase、CNKI等数据库中的中英文文献。采用循证医学方法,对采纳的原始研究数据进行异质性检验,计算其汇总加权灵敏度和特异度,然后建立受检者工作特征曲线(SROC),通过曲线下面积评估双源CT双能量技术在诊断孤立性肺结节中的价值,并使用QUADAS条目进行文献质量评价。结果:经检索总共采纳3篇文献,其中2篇为中文,1篇为英文。按照固定效应模型加权定量合并,以20Hu作为阈值,利用SPN碘分布图CT值评估其良恶性的灵敏度和特异度分别为87%和73%;利用SPN强化值评估其良恶性的灵敏度和特异度分别为75%和68%。结论:双源CT双能量技术在孤立性肺结节的研究中具有较好的临床应用价值。Objective. To assess the clinical value of dual energy CT technology in the research of solita- ry pulmonary nodules with meta-analysis. Methods: Dual source CT, dual energy, and solitary pulmonary nodule were selected as the keywords. A collection in Cochrane library, PubMed, Embase, CNKI was performed to identify related Chinese and English documents from January 2006 to August 2012. The original data were performed heterogeneity test with the evidence-med- ical method. The weighted sensitivity and specificity were calculated, as well as the summary re- ceiver operating characteristic (SROC) curve. Further estimate of the value of dual-source CT in the diagnosis of solitary pulmonary nodules was performed by calculating the area under the curve. All articles were evaluated by QUADAS items. Results: A total of 3 studies were adopted for meta-analysis, including two Chinese papers and one English paper. According to the fixed effects model, diagnostic efficiency with a cut off of 20 Hu for malignancy by using CT value on i- odine enhanced image was comparable to that by using the degree of enhancement (sensitivity, 87% versus 75% ;specificity, 73% versus 68% ,respectively). Conclusion: Dual energy CT tech- nology in the study of solitary pulmonary nodules has a good clinical value.
