老年男性血清胎球蛋白A水平与肾小球滤过率的关系

摘　　要：目的:探讨老年男性血清胎球蛋白A水平与肾小球滤过率的关系。方法:选取我科住院的老年男性患者(年龄≥60岁)98例,平均年龄(78.9±6.2)岁。采用酶联免疫吸附(ELISA)法测定血清胎球蛋白A水平。用肾脏病膳食改良试验(MDRD)公式估算肾小球滤过率(eGFR)。根据eGFR及胎球蛋白A水平分组,评价老年男性血清胎球蛋白A水平与肾小球滤过率的关系。结果 :胎球蛋白A水平与年龄、血肌酐(SCr)及血尿素氮水平均呈负相关(r分别为-0.227、-0.241、-0.229,均P<0.01),与eGFR及血钙均呈正相关(r分别为0.303和0.251,均P<0.01)。肾功能较好组[eGFR≥60 mL/(min·1.73 m2)]的胎球蛋白A水平较肾功能较差组[eGFR<60 mL/(min·1.73 m2)]的胎球蛋白A水平高[(41.95±7.55)mg/L比(38.33±6.95)mg/L,P<0.05]。胎球蛋白A水平较高组的肾功能状态好于胎球蛋白A水平较低组[(80.92±17.43)mL/(min·1.73 m2)比(65.47±22.94)mL/(min·1.73 m2),P<0.01]。结论:老年男性胎球蛋白A水平与肾功能状态相关,随着年龄的增长及肾小球滤过率的下降而下降。Objective To study the correlation between serum fetuin-A level and glomerular filtration rate （GFR） in elderly men. Methods A total of 98 elderly men （≥60 years） were enrolled. Serum level of fetuin A was measured by enzyme-linked immunosorbent assay （ELISA）. The GFR was estimated（eGFR） by the modification of diet in renal disease （MDRD） equation. Results Serum fetuin A level was negatively correlated with age, serum creatinine （SCr） and blood urea nitrogen （BUN） levels （r was -0.227, -0.241 and -0.229, P〈O.01） , and was positively correlated with serum calcium level and eGFR （r was 0.303 and 0.251, P〈0.01）. The serum fetuin A level of good renal function group [eGFR≥60 mL/ （min. 1.73 m2）] was significantly higher than that of poor renal function group [eGFR〈60 mL/（min. 1.73 m2）, P〈0.05]. Comparing with the higher serum fetuin level group, the lower serum fetuin level group had higher BUN and SCr level and lower eGFR. Conclusions Level of serum fetuin A is correlated with renal function in elderly men. Serum fetuin level is decreased with the increasing of age and decreasing of eGFR.
