摘  要:目的探讨踝部小剂量对比剂预试验技术在多层螺旋CT下肢动脉血管成像中的作用。方法选择40例临床怀疑下肢动脉疾病的患者,随机分成试验组和对照组。试验组采用2次小剂量对比剂预试验注射技术,根据踝关节水平胫前动脉和腹主动脉的动态扪捕达峰时间确定CTA的扫描延迟时间及扫描持续时间。对照绀采用示踪法技术。比较2组患者的CTA图像质挝差片。结果双侧胫前动脉达峰时间(TTI)差肄较大,时间范H爿和平均时间分别为28.0~60.0s和(49.1S±7.9)s。腹主动脉-胫前动脉的通过时间(Tt)为10.0~31.0S,平均(18.8±5.5)s,T1的个体间差异大。小腿动脉段试验组的动脉边缘光滑度、CT值及足部动脉分支级别均优于对照纰(P〈0.05)。结论应用踝部小剂量对比剂预试验技术可以准确把握下肢动脉达峰时间,特别是膝以下小动脉的达峰时间,从而获得高质量的下肢动脉图像。Objective To investigate the clinical value of the low-dose contrast agent test bolus technique at ankle level on multislice CT in lower extremity arterial angiography. Methods Forty cases suspected lower extremity arterial disease, who were randomly divided into test group and control group. The test group used two low dose test bolus injection technique, according to the peak time of anterior tibial artery at ankle level and abdominal aorta, to determine the CTA scan delay time and scan duration. The control group used bolus-tracking technique. The quality of images in both groups was compared. Results Time to peak of bilateral anterior tibial artery was quite different, the time range was 28.0-60.0 s,and the average time was (49.1±7.9) s. Abdominal aorta anterior tibial artery transit time(Tt)was 10.0-31.0 s, the average time was (18.8±5.5) s, a larger individual variation in Tt was observed. The clarity of artery border, CT value in leg artery and the foot artery branch level in test group, was better than the control group. Conclusion The application of low-dose of contrast agent test bolus technique at ankle level can accurately grasp the lower extremity arterial peak time, especially in below knee arteries, so as to obtain high quality images.
