自发性乙状结肠破裂的MSCT诊断

摘　　要：目的探讨自发性乙状结肠破裂的MSCT（multi—slice CT）诊断价值。方法同顾性分析经手术病理证实的9例白发性乙状结肠破裂的MSCT表现。结果MSCT轴位扫描乙状结肠破裂口直接@示2例，未显示7例；9例破裂口周围均见肠外粪块影，不成形8例,成形1例；粪块周围有炎性渗出改变7例，渗出不明显2例；破口周围乙状结肠肠壁轻度增厚、水肿；病灶周同局限性小气泡及盆腔少量积液2例．全腹散在游离气体及腹腔多处积液7例。结论MSCT是自发性乙状结肠破裂的最佳检查方法。“肠外粪块”征是其特征性CT表现，对明确诊断有重要价值。Objective To investigate the value of muhi slice CT （MSCT） in the diagnosis of spontaneous rupture of the sigmoid colon. Methods MSCT findings of 9 patients with spontaneous rupture of the sigmoid colon were retrospectively evaluated, which were proved by pathology. Results Of the 9 patients scanned by MSCT, 2 rupture cuts were displayed on axial image, the others were not found. The feces outside the rupture colon were found in all 9 cases, and i case was shaped feces, the others were unshaped one. 7 patients were showed plenty of inflammatory exudation around the feces, and the sign was not obvious in the other 2 cases. The wall of sigmoid colon adjacent rupture holes thickened and became edematous. Small air bubbles near the rupture cuts and a lit tie amount of pelvic effusion were showed in 2 cases. Free air and effusion in variable locations of the abdominal cavities were found in 7 cases. Conclusion MSCT is the best way to demonstrate the spontaneous rupture of the sigmoid colon. The sign of ＂feces out of the colon wall＂ is specific characteristic sign on CT, which plays an important role in differential diagnosis of this disease.
