摘 要:目的 通过观测复苏犬心肌组织ATP酶活性及离子含量的变化 ,探讨复苏后心肌损伤的机制。方法 电击致犬室颤心跳呼吸骤停 ,15min后进行标准复苏 ,测量心脏停跳 15min后 ,复苏后 0 5、2、4h时心肌中Na+、K+、Ca2 +含量的变化。结果 与正常组相比 ,心脏停跳 15min时 ,心肌中Na+-K+ATP酶、Ca2 +ATP酶活性即明显下降 (P <0 0 1vsP <0 0 5 ) ;复苏后 0 5h ,Ca2 +ATP酶活性从正常组的 5 12± 0 40 μmol·mg-1·h-1降到最低点 2 73± 0 40 μmol·mg-1·h-1,同时心肌组织中Na+、Ca2 +含量显著升高 (P <0 0 1vsP <0 0 1) ,K+含量降低 (P <0 0 1) ;复苏后 2h ,Na+-K+ATP酶活性降至最低点 (P <0 0 1) ,心肌中Na+、Ca2 +含量升高更明显 ;复苏后 4h ,心肌中Ca2 +含量从正常组的 2 6 5± 0 2 7μmol·g-1·dw升至高峰 4 33±0 41μmol·g-1·dw ,其它各种变化开始恢复。各实验组与相应的阴性对照组比较 ,结果与上述类似。结论 复苏后心肌ATP酶活性及离子含量的变化可能参与了复苏后心肌损伤的发生 ,其中Ca2Objective To investigate the mechanism of myocardial injury post-resuscitation by way of observing changes of ATPase activity and ion contents of myocardium in resuscitation dogs.Methods Dogs were given electric shock with ventricular fibrillation cardiac arrest (CA) and normally resuscitated after 15 minutes. Na +-K +ATPase and Ca 2+ ATPase activity, Na +、K +、Ca 2+ contents of myocardium were measured at 15 minutes after CA, 0.5, 2, 4 h post-resuscitation respectively.Results Myocardial Na +-K +ATPase and Ca 2+ ATPase activity of CA group decreased significantly( P <0 01 vs P <0 05) compared with that of the normal group 5 12±0 40 μmol·mg -1 ·h -1 to the lowest value 2 73±0 40 μmol·mg -1 ·h -1 . Meanwhile, Na +,Ca 2+ contents of myocardium increased significantly( P <0 01 vs P <0 01),K + content reduced markedly ( P <0 01).At 2 h post-resuscitation, Na +-K +ATPase activity reached the lowest level, but Na +,Ca 2+ contents were still higher than that of normal group. At 4 h post-resuscitation, myocardial Ca 2+ content increased from normal 2 65±0 27 μmol·g -1 ·dw to peak value 4 33±0 41 μmol·g -1 ·dw and all other indexs began to restore. The result of each experimental group′s index compared with responding control group′s is similar to that of each experimental group′s compared with the normal group′s.Conclusion The reduction of ATPase activity and changes of ion contents could participate in the myocardial injury post-resuscitation, the increase of Ca 2+ content among them could be crucial.
