摘  要:目的 ①研究急性脑外伤血清神经元特异性烯醇化酶 (NSE)的含量变化。②探讨中—重型脑损伤血清NSE改变的时间过程与预后的关系。方法 用ELISA法测定 5 1例急性脑外伤病人血清NSE水平 ,对部分中—重型病人进行连续测定。结果 轻型组血清NSE水平与正常对照组比较无明显升高 (P >0 0 5 ) ,中型组和重型组均高于轻型组 (P <0 0 0 1) ,重型组亦高于中型组 (P <0 0 5 ) ;中型组和重型组中的存活者首次血清NSE值即为峰值 ,以后逐渐下降 ,中型组伤后第 2天、重型组伤后第 4天接近正常 ;伤后近期死亡者NSE首次测定并非峰值 ,以后进行性增高 ,直至死亡。Objective ①To study serum levels of neuro-specific enolase(NSE)in patients with acute head injury.②To investigate the time course of serum NSE after moderate and severe head injury in correlation to outcome.Methods Serum NSE levels were measured in 51 patients with acute head injury by ELISA.Serial serum NSE levels were measured in part of patients with moderate and severe head injury.Results A significant elevation was observed in serum NSE levels in patients with moderate and severe head injury,relative to patients with minor head injury( P <0 001).A significant difference was also noted between NSE levels in patients with severe head injury and those with moderate head injury( P <0 05).In surviving patients with moderate and severe head injury,the highest levels of NSE were found in the first samples.The values declined toward normal levels within 2 days in patients with moderate head injury and 4 days in patients with severe head injury.In patients who died within 3 days,high levels of NSE persisted or even increased until death ensued.Conclusions These results support the use of serum NSE for early estimation of the degree of brain injury and prognosis of patients with acute head injury.
