经皮冠状动脉介入术治疗对女性冠心病患者长期生活质量和预后的影响

摘　　要：目的探讨经皮冠状动脉介入术(PCI)对女性冠心病患者长期生活质量(QOL)和预后的影响。方法选择武汉市中心医院确诊为冠心病的女性患者212例,根据是否进行PCI治疗,分为手术组(124例)和药物组(98例)。采用SF-36量表评价两组患者随访6、12个月的QOL评分,用Log-rank检验比较12个月的联合终点事件(因心脏原因入院和全因死亡)的发生率。结果手术组和药物组分别有114例和87例完成随访。随访6、12个月,手术组QOL评分[(13.77±2.11)、(19.57±3.00)分]高于药物组[(7.83±3.58)、(10.79±3.92)分],差异均有高度统计学意义(P<0.01)。手术组联合终点事件发生率为14.91%(17/114),药物组发生率为31.03%(27/87),生存曲线显示两组终点事件发生率差异有高度统计学意义(P<0.01)。结论 PCI手术治疗能够显著提高女性冠心病患者的QOL评分,改善预后。Objective To investigate the impact of PCI on quality of life （QOL） and prognosis in female patients with coronary heart disease （CHD）.Methods Total 212 cases of female CHD patients in Central Hospital of Wuhan City were grouped into operation group （124 cases） and drug group （98 cases） according to the treatment method.QOL score was evaluated in the two groups by 6,12 months＇ following-up with SF-36 scale,and the 12 months＇ combined end point events occurrence rate （rehospital because of cardiac cause and all-cause death） was compared with Log-Rank test.Results There were 114 paitents in PCI group and 87 patients in drug group finishing the following-up.QOL scores in operation group 6,12 months after the following-up [（13.77±2.11）,（19.57±3.00） scores] were higher than those in drug group [（7.83±3.58）,（10.79±3.92） scores],the differences were statistically significant （P ＜ 0.01）.The rate of end point events in operation group [14.91％ （17/114）] was low er than that in drug group [31.03％ （27/87）],the difference was statistically significant （P ＜ 0.01） according to the survivorship curve.Conclusion PCI surgery can significantly improve the QOL scores and prognosis of female CHD patients.
