2型糖尿病患者降糖治疗过程中出现低血糖的影响因素分析

摘　　要：目的 探讨2型糖尿病患者在降糖治疗过程中出现低血糖的原因,并对出现低血糖的影响因素进行分析归纳,为临床实践提供参考. 方法选择2010年2月-2013年3月就诊我院收治的100例2型糖尿病患者作为观察对象,将药物治疗中出现低血糖的患者（50例）设为观察组,未发生低血糖的患者（50例）设为对照组,采集患者临床资料,检测空腹血糖,糖化血红蛋白等指标,分析出现低血糖的影响因素. 结果两组患者在年龄、病程、BMI及CR方面差异显著（P＜0.05）;经Logistic回归分析,BMI（OR=0.648,P=0.017）和年龄（OR=1.812,P=0.010）是低血糖的危险因素,其中BMI是2型糖尿病发生低血糖的保护性因素,与低血糖的发生呈负相关,而年龄因素则与之正相关. 结论 2型糖尿病治疗过程中的低血糖是多种因素共同作用的结果,减少危险因素并及时调整治疗方案是降低低血糖发生的关键,建议患者熟练掌握低血糖特点和解决手段,避免低血糖发生.Objective To investigate the factors of hypoglycemic in the process of type 2 diabetes patients, and analyze and summarize the factors which affect the hypoglycemia, to provide the reference for clinical practice. Methods A total of 100 type 2 diabetes patients from February 2010 to March 2013 were selected as the observation object, the patients with hypoglycemia drug treatment （50 cases）for the observation group ,there were no hypoglycemia patients （50 cases） were set the control group, collectted the clinical data, fasting blood glucose,glycosylated hemoglobin, ana- lyzed the influence factors of hypoglycemia. Results Two groups were significant difference in age, course of disease, BMI and CR （P〈0.05）;Logistic regression analysis, BMI（OR=0.648, P=0.017） and age （OR=1.812, P=0.010） were risk factors of hypoglycemia,where BMI was a protective factor for type 2 diabetes,hypoglycemia,and negatively cor- related with the occurrence of hypoglycemia, and age factors and positive correlation, both were the risk factors of hypoglycemia. Conclusion The treatment of type 2 diabetes during hypoglycemia is the result of many factors,the reduction of risk factors and adjust the treatment plan is the key of reducing hypoglycemia,advise patients to master thecharacteristics of hypoglycemia and solution, avoid hypoglycemia.
