快速检测NT—proBNP在老年急性呼吸困难鉴别诊断中的应用

摘　　要：目的研究快速检测N末端脑利钠肽原（NT-proBNP）在老年急性呼吸困难鉴另1诊断中的应用价值。方法选取76例以急性呼吸困难为主诉的住院年龄〉60岁的老年患者，剔除由外伤、心包填塞和急性冠状动脉综合征等所致的呼吸困难者，另选取30例正常人作为对照组。利用德国罗氏公司的心脏标志物检测仪，检测呼吸困难患者NT-proBNP的浓度，48h内进行超声心动图检查。结果心源性呼吸困难患者NT-proBNP的浓度明显高于非心源性呼吸困难患者，心源性呼吸困难组NT-proB—NP在心功能不同级另q间差异有统计学意义（P〈O．01）。结论对于老年呼吸困难患者，床边检测全血NT-proBNP浓度，简便快速、敏感性高、特异性强，可作为鉴别老年呼吸困难患者的参考性指标之一。Objective To research the application value of the rapid detection of the NT-proBNP in elderly acute dyspnea differ- ential diagnosis. Methods Select 76 patients hospitalized with acute dyspnea for complaint, age〉 60 elderly patients. Removed by trauma, acute coronary syndromes and cardiac tamponade due to breathing difficulties, the other choose 30 eases of normal people as control group. Take advantage of Germany roche cardiac makers detector, detect the concentration of NT-proBNP in patients with breathing difficulties,echocardiography within 48h. Results Cardiac dyspnea NT-proBNP in patients with concentration signifieant- ly higher than patients without cardiac dyspnea,cardiac dyspnea group NT-proBNP in cardiac function at different levels have sta- tistical significance （P〈0.01）. Conclusion For elderly patients with dyspnea, bedside detection of whole blood concentration of NT-proBNP,simple and rapid,high sensitivity, strong specificity,can be used as identification of one of reference index of elderly patients with dyspnia.
