摘　　要：目的　研究经皮穿刺腰椎间盘切除术和胶原酶化学溶解术联合治疗腰椎间盘突出症的疗效。方法　将312例腰椎间盘突出症病人 ,随机分为 3组 ,分别采用经皮穿刺腰椎间盘切除术、胶原酶化学溶解术和二者联合治疗方法进行治疗。术后进行随访和疗效判定。结果　接受联合治疗的病人的疗效与接受单一方法治疗的病人的疗效有着显著的差异 (P <0 .0 5 ) ,联合治疗的疗效明显优于后者。结论　联合方法具有疗程短、见效快 ,安全 ,有效率高等优点 ,是一种理想的治疗腰椎间盘突出症的方法 ,值得推广应用。Objective To investigate the therapeutic effect of percutaneous lumbar diskectomy (PLD) combining with chemonucleolysis (CNL) by injection of collagenase in the treatment of lumbar disc herniation.Methods 312 patients with Lumbar disc herniation were classified into three groups randomly, and were treated respectively with PLD,CNL and PLD combining with CNL.All patients were followed up after the operation from 3m,6m,12m,24m. According to the improved standard that Macnab made,the patients' therapeutic effects were judged.Results The therapeutic effects of the patients who were treated with PLD combining with CNL were obviously different from those who were treated with single method( P <0.05). Joint treatment was superior to single treatment.Conclusion Joint treatment has many advantages, such as short therapeutic course, fast effect, safety, high effective effect, etc. So it is an ideal method to treat lumbar disc herniation and was worth using widely.
