TESSYS技术与开放手术治疗腰椎间盘突出症的效果及安全性比较

摘　　要：目的比较TESSYS技术与传统开放手术治疗腰椎间盘突出症的效果及安全性。方法选取2009年3月～2012年9月本院收治的腰椎间盘突出症患者78例，将选择经皮椎间孔镜下TESSYS技术治疗的43例作为TESSYS组，选择传统开放手术治疗的35例作为开放组。术前、术后及末次随访时应用视觉模拟评分（VAS）评估疼痛程度，应用日本骨科协会（JOA）评分评估腰椎功能改善情况。比较两组的手术时间、出血量和并发症发生率。结果两组患者术前VAS和JOA评分差异无统计学意义（P〉0．05），术后VAS均显著下降（P〈0．05），JOA评分均显著升高（P〈0．05），且两组间VAS和JOA评分差异无统计学意义（P〉0．05）。TESSYS组的手术时间短于开放组、出血量少于开放组、并发症发生率低于开放组（P〈0．05）。结论与传统开放手术相比，TESSYS技术治疗腰椎间盘突出症疗效相当。且安全性更高，具有手术时间短、出血少、并发症少的优点。Objective To compare the effect and safety of transforaminal endoscopic spine system （TESSYS） and open operation in the treatment of lumbar disc herniation.Methods Seventy-eight cases of patients with lumbar disc hernia- tion in our hospital from March 2009 to September 2012 were selected,of whom 43 cases recieved percutaneous inter- vertebral foramen microscopically TESSYS technical treatment were as TESSYS group,35 cases received open opera- tion were as open group.Their pain degree and lumbar function improvement was assessed using VAS and JOA reoper- atively,postoperatively and at the end of follow-up sessions,respectively.Besides,operation time,bleeding volume and complication odds of them was compared.Results There was no significant difference in VAS and JOA score preopera- tively between two groups （P〉O.05）,VAS score of both groups was decreased （P〈0.05）,and JOA score was increased （P〈 0.05）.There was no significant difference in VAS and JOA score postoperatively between to groups （P〉0.05）.While, compared with open group,the operation time of TESSYS group was shorter,bleeding volume was less and complication odds was lower （P〈0.05）.Conclusion Compared with open operation,TESSYS technology can bring the similar effect in the treatment of lumbar intervertebral disc herniation,and it takes shorter operation time,less bleeding,less complication.
