摘 要:目的应用三维超声任意平面成像及断层超声显像技术探讨胎儿肾盂扩张的影响因素及不同转归,评价胎儿肾盂扩张的程度,指导临床干预。方法对30例二维超声初检单侧肾盂前后径≥10 mm的胎儿进行随访,应用三维任意平面成像及断层超声显像技术观察、测量并记录其肾盂前后径、肾实质厚度及肾动脉的阻力指数(RI)。结果 30例胎儿肾积水中,10 mm≤肾盂前后径<15 mm的胎儿中,肾实质厚度≤5 mm者5例,出生后消失2例,手术1例;肾实质厚度>5 mm者21例,出生后消失18例,手术0例;在本组中,RI≥0.7者4例,出生后消失1例,手术1例;RI<0.7者22例,出生后消失19例,手术0例。肾盂前后径≥15 mm的胎儿中,肾实质厚度≤5 mm者3例,出生后消失0例,手术2例;肾实质厚度>5 mm者1例,出生后消失1例,手术0例;在本组中,RI≥0.7者3例,出生后消失0例,手术2例;RI<0.7者1例,出生后消失1例,手术0例。结论产前超声检查示肾盂前后径≥15 mm,肾实质厚度≤5 mm且RI≥0.7的胎儿往往存在病理性梗阻,应高度重视。三维超声技术能够迅速、准确地获得诊断肾盂扩张的标准切面。Objective To explore the influencing factors and different prognosis of fetal pyelectasis by arbitrary planar imaging (API) and tomographic ultrasound imaing (TUI),evaluate the extent of fetal pyelectasis and direct the clinical intervention.Methods Thirty fetuses with anteroposterior diameter of renal pelvic (APD) ≥ 10 mm were fllowed up,and API and TUI was used to observe,measure and record the APD,the thickness of renal parenchyma and resistance index of renal artery (RI).Results In 30 cases of fetal hydronephrosis,10 mm ≤ APD ≤ 15 mm,renal parenchymal thickness ≤ 5 mm were 5 cases,2 cases disappeared after birth,one case was taken surgery;renal parenchymal thickness 〉 5 mm were 21 cases,18 cases disappeared after birth,0 case was taken surgery;in this group,RI ≥ 0.7 were 4 cases,1 case disappeared after birth,one case was taken surgery;RI 〈 0.7 were 22 cases,19 cases disappear after birth,0 case was taken surgery.APD ≥ 15 mm,renal parenchymal thickness ≤ 5 mm were 3 cases,0 case disappeared after birth,two cases were taken surgery;renal parenehymal thickness 〉 5 mm was 1 case,1 case disappeared after birth,0 case was taken surgery;in this group,RI ≥ 0.7 were 3 cases,0 case disappeared after birth,two cases were taken surgery;RI 〈 0.7 was 1 case,1 ease disappeared after birth,0 case was taken surgery.Conclusion The fetus with APD 〉I 15 mm, thickness of renal parenchyma 〈 5 mm and RI ≥ 0.7 should be highly valued.Three-dimensional ultrasound technol- ogy can quickly and accurately obtain the standard section for fetal pyelectasis.
