摘  要:目的:比较替米沙坦、氨氯地平以及二者联用治疗肾性高血压大鼠左室重构的作用及其对瘦素、一氧化氮(NO)含量的影响。方法:建立两肾一夹高血压大鼠模型,健康雄性SD大鼠50只,随机分为5组,每组10只,两肾一夹高血压模型组(2KIC组)、假手术组、替米沙坦(10 mg·kg-1·d-1)给药组、氨氯地平(2.5 mg·kg-1·d-1)给药组、替米沙坦(10 mg·kg-1·d-1)+氨氯地平(2.5 mg·kg-1·d-1)给药组;每周测1次尾动脉压。20周后采集标本,(1)测定左室质量指数(LVMI);(2)观察心肌电镜超微结构;(3)硝酸还原酶法检测血清NO浓度;(4)酶联免疫法检测血清瘦素含量。结果:与假手术组相比,2KIC组大鼠SBP、LVMI、血清瘦素含量明显升高(P<0.01),血清NO浓度明显降低(P<0.01);各单用药组与2KIC组相比SBP、LVMI、血清瘦素含量明显降低(P<0.01),血清NO浓度明显升高(P<0.01);联合用药组与两单用药组相比更显著的降低SBP、LVMI、血清瘦素含量(P<0.01),升高血清NO浓度(P<0.01)。心肌电镜结果显示联合用药组更显著地改善2KIC高血压大鼠左室重构。结论:替米沙坦和氨氯地平均能有效地改善高血压左室重构,二者联用效果更明显。其机制可能与降低血清瘦素含量,升高血清NO浓度相关。OBJECTIVE To compare the effects of telmisartan and(or) amlodipine on the reversal left ventricular remodeling and the levels of leptin and nitric oxide in two kidney one clip renal hypertensive rats. METHODS The model of two kidney one clip renal hypertensive rats was established and 50 healthy male SD rats were randomly divided into 5 groups(n = 10) ; two kid- ney one clip renal hypertensive rats (2KIC) group, sham operated group, telmisartan( 10 mg. kg- 1. d- 1 ) group, amlodipine(2. 5 mg. kg-1 .d-1 )group, telmisartan(10 mg- kg-1 .d-1 ) + amlodipine(2. 5 mg. kg 1 .d-1 )group. The blood pressure was de- tected once a week. After 20 wk,the rats were sacrificed and the specimens were collected, left ventricular mass index(LVMI) was assessed,myocardial ultrastructural changes were observed by electron microscope. Nitric oxide (NO) level in serum was examined by nitrate reductase assay. The level of leptin(LEP) in serum was measured by enzyme linked immunosorbent assay (ELISA). RESULTS Compared with sham-operated group, SBP, LVMI and LEP level increased significantly in 2KIC group (P〈0. 01), but the level of NO significantly decreased(P〈0. 01). Compared with 2KIC group, SBP, LVMI and LEP level decreased significantly in telmisartan group and amlodipine group(P〈0. 01), but NO level significantly increased(P〈0. 01). The combined medication group reduced SBP, LVMI and LEP level more significantly(P〈0. 01)than each single drug group, meanwhile increased the level of NO in serum more significantly(P〈0. 01). Myocardial electron microscopy results showed that the combined treatment group reversed left ventricular remodeling more effectively in 2KIC hypertensive rats. CONCLU- SION Therapy ameliorated left ventricular remodeling, the effect of combined telmisartan with amlodipine on the reversal of left ventricular hypertrophy was more pronounced than that by single therapy.
