摘 要:目的 研究国产依普拉封片 (ipriflavone)防治绝经后骨质疏松症疗效的生化学监测及疗效机制。方法 绝经后骨质疏松症 5 4例 ,其中治疗组 30例服用国产依普拉封片合用钙尔奇 D治疗 6个月 ,对照组 2 4例单独服用钙尔奇 D6个月 ,检测两组用药前后骨代谢生化指标。结果 治疗组结果表明骨吸收的指标 :甲状旁腺素 (PTH)、抗酒石酸盐酸性磷酸酶 (TRAP)、尿胶原交联 /肌酐比值 (Crosslaps/Cr)均明显降低 ;骨形成 /骨转换的指标 :碱性磷酸酶 (ALP)、骨钙素 (BGP)、I型前胶原羧基端前肽(PICP)亦下降 ;骨重建刺激因子 :胰岛素样生长因子 1(IGF 1)显著升高。对照组 ,TRAP也有轻度下降。结论 依普拉封片能明显的影响骨代谢 :抑制骨吸收 ,降低骨转换 ,减少骨量丢失 ,治疗 1个月即可出现作用 ;刺激骨重建 ,增加骨密度 ,能够有效地防治绝经后骨质疏松症。生化指标可用于疗效的早期监测。OBJECTIVE To monitor the change of biochemistry in patients with postmenopausal osteoporosis after intaking ipriflavone and discuss its action mechanism.METHODS 54 patients with post menopause osteoporosis were divided into 2 groups:30 patients were administrated with ipriflavone and Caltrate D for 6 months.24 patients in control group were only given Caltrate D for 6 months. The bone biochemical indexes were monitored before and after the treatment.RESULTS PTH, TRAP and Crosslaps/Cr, the indexes of bone absorption, decreased markedly; ALP, BGP and PICP, the indexes of bone formation/turnover, decreased markedly. IGF 1, strong stimulator of bone rebuild, significantly increased.In the control group,Ca 2+ in blood and TRAP lightly decreased.CONCLUSION The results indicated ipriflavone decreased the bone absorption and bone turnover rate, prevented bone loss.The efficacy appeared in 1st month after administration;stimulating bone rebuild,and raise BMD.It is effective in preventing and treating postmenopausal osteoporosis. Biochemistry index can be used to monitor the effect of treatment early. KEY WORDS:ipriflavone;postmenopausal osteoporosis;bone .40,with peak absorption at 285 nm.its molecular weight was 11 047 on Sephadex G 200.The UPL was nonspecific in agglutination for any type of human erythrocytes,and the hemagglutinating activity was equal.The agglutination for rabbits was the highest,among erythrocytes of humans(A,B,AB,O),rabbits,common carp and crucian carp.The hemagglutinating activity for rabbit could not be inhibited by D fructose, D galactose,glucose,sucrose,mannan,gamma globulin(human)and egg albumin,but inhibited by bovine thyroglobulin and the minimum inhibitory concentration was 6 20 g/L.This lectin had the maximum hemagglutinating activity at pH 6 50-9 51,with activity at pH4 0-10 14.The hemagglutinating activity was not affected by heating for 60 min at 85℃.So it had high heat resistance.
