芒针深刺阿是穴治疗中风后肩痛临床观察

摘　　要：目的观察芒针深刺阿是穴治疗中风后肩痛的临床疗效及安全性。方法将60例患者随机分入治疗组和对照组,分别给予芒针深刺阿是穴及毫针针刺"肩三针"治疗,治疗前后采用视觉模拟评分(VAS)、肩痛频度评估疼痛程度,修订的Barthel指数(MBI)评估日常生活能力,治疗结束后评价两组患者治疗满意度。结果依据VAS评价,治疗组总有效率为80.0%,对照组为43.3%,两组疗效比较差异有统计学意义(P<0.01);治疗后两组VAS评分、MBI评分较治疗前均明显改善(P<0.01),组间VAS评分比较差异有统计学意义(P<0.01),而组间MBI评分比较差异无统计学意义(P>0.05);治疗组治疗前后肩痛频度评分差异有统计学意义(P<0.01),而对照组无统计学意义(P>0.05)。结论芒针深刺阿是穴可明显减低中风后肩痛患者的疼痛程度、频度,提高日常生活能力。Objective To observe the clinical efficacy and safety of deep acupuncture at Ashi points with elongated needle in treating post-stroke shoulder pain. Method Sixty patients were randomized into a treatment group and a control group, to respectively receive deep acupuncture at Ashi points with elongated needles and acupuncture at ＂three shoulder points＂ [Jianyu （LI15）, Jianqian （extra）, and Jianzhen （SI9）] with filiform needles. The Visual Analogue Scale （VAS） and shoulder pain frequency were employed to evaluate pain intensity, and the Modified Barthel Index （MBI） was used to evaluate the activity of daily living before and after treatment. The patient satisfaction was assessed at the end of treatment. Result According to the VAS, the total effective rate was 80.0% in the treatment group versus 43.3% in the control group, and the difference was statistically significant （P 〈0.01）; aider treatment, the two groups both had significant improvements in VAS and MBI scores （P〈0.01）, and the inter-group difference in VAS score was statistically significant （P〈0.01）, but the inter-group difference in MBI score was statistically insignificant （P〉0.05）; the treatment group had marked change in the shoulder pain frequency score after treatment （P〈0.01）, while the control group didn＇t achieve a significant change （P〉0.05）. Conclusion Deep acupuncture at Ashi points with elongated needles can significantly reduce the intensity and frequency of post-stroke shoulder pain, and enhance the activity of daily living, with content safety.
