针刺手厥阴经穴对脑卒中手软瘫期手功能恢复的影响

摘　　要：目的观察针刺手厥阴经穴对脑卒中手软瘫期手功能恢复的影响。方法将60例脑卒中手功能软瘫期患者(Brunnstrom分级Ⅰ级)随机分为观察组30例和对照组30例,所有患者均在常规治疗的基础上,观察组予针刺手厥阴经穴,当患肢手渡过软瘫期,功能恢复达到Brunnstrom分级Ⅱ级以上(包括Ⅱ级)时,改取上肢阳经穴针刺;对照组予针刺手阳明经穴,疗程20 d,治疗前后分别予患手Brunnstrom运动功能评定。结果两组治疗后患手Brunnstrom运动功能评分较治疗前均有明显提高(P<0.01),观察组手功能评分及疗效明显优于对照组(P<0.05)。结论脑卒中患者于手功能障碍软瘫期针刺手厥阴经穴能改善手运动功能且疗效优于针刺手阳明经穴。Objective To observe the effect of acupuncture at the Pericardium Meridian on the recovery of hand function at flaccid paralysis stage after cerebral stroke. Method Sixty post-stroke patients at flaccid paralysis stage （Brunnstrom degree II ） were randomized into an observation group of 30 cases and a control group of 30 cases. In addition to the basic treatment, the observation group also received acupuncture at points from the Pericardium Meridian. The points would be replaced by those from the yang meridians of upper limbs when the patients＇ hand function reached Bmnnstrom degree Ⅱ or above. The control group received acupuncture at the Large Intestine Meridian in addition to the basic treatment. 20-day treatment was taken as a course, and the affected hands were evaluated by using Brunnstrom Approach before and after treatment. Result The Brunnstrom scores increased significantly in both groups after treatment （P^0.01）, and the hand function score and therapeutic efficacy in the observation group were markedly better than those in the control group （P〈0.05）. Conclusion For patients with post-stroke hand dysfunction at flaccidity stage, acupuncture at the Pericardium Meridian can produce better therapeutic efficacy than acupuncture at the Large Intestine Meridian in improving hand function.
