尿激酶型纤溶酶原激活物受体对子宫内膜异位症发生发展及临床分期的作用研究

摘　　要：目的探讨尿激酶型纤溶酶原激活物受体（urokinase plasminogen activator receptor，uPAR）对子宫内膜异位症（endometriosis，EMT）发生发展的作用。方法应用免疫组化方法检测uPAR蛋白在40例EMT患者异位内膜及在位内膜，30例正常子宫内膜的表达。结果uPAR蛋白主要定位于细胞浆，在异位内膜、在位内膜、对照组内膜腺上皮细胞及间质细胞均表达，且在血管内皮细胞呈阳性表达。uPAR蛋白在异位内膜、在位内膜及对照组内膜表达腺上皮细胞高于间质细胞（P〈0．05），在腺上皮细胞表达异位内膜高于在位内膜及对照组内膜（P〈0．05）。uPAR蛋白在异位内膜Ⅰ～Ⅱ期与Ⅲ～Ⅳ期的表达比较无统计学意义（P〉0．05）。结论uPAR在异位内膜腺上皮细胞高表达可能促使其转移、粘附、侵袭、生长，导致EMT的发生发展；uPAR的作用研究对EMT的临床治疗有一定的指导意义。[ Objective ] To investigate the function of uPAR in genesis and development of endometriosis. [ Methods ] Immunohistochemistry method was applied to detect the expression of uPAR protein in 40 cases of eetopic endometrium, 40 cases of eutopic endometrium and 30 cases of endometrium without endometriosis. [ Resluts ] UPAR protein was expressed in stromal and epithelial cells from ecpotic, eutopie and normal endometrium, and also detected positively in vascular endothelial cells, uPAR protein was significantly higher from epithelial cells than from stromal cells in ecpotic,eutopic and normal endometrium （P〈0.05）. uPAR protein in epithelial cells was increased in ectopic endometrium compared with eutopic and normal endometrium （P〈0.05）.There was no statistical significance between the level of uPAR in stage Ⅰ - Ⅱ and stage Ⅲ- Ⅳ of ectopie endometrium（P〉0.05）. [ Conclusion ] The overexpression of uPAR protein in endometriotic stromal cells might be of importance for the migratory,adherance,invasion and growth,which maybe result in endomrtriosis in the end.Study on the effect of uPAR has a certain guiding significance for clinical treatment of endometriosis.
