整颈三步九法推拿治疗颈性眩晕患者60例临床观察

摘　　要：目的观察整颈三步九法推拿治疗颈性眩晕的临床疗效。方法将120例颈性眩晕患者随机分为治疗组和对照组各60例,治疗组给予整颈三步九法推拿治疗,对照组给予传统推拿手法治疗,两组均每次治疗20min,隔天治疗1次,3次为1个疗程,治疗2个疗程后观察临床疗效,并于治疗前后进行头晕问卷调查表(DHI)和生存质量量表(SF-36)评分。结果治疗组临床疗效总有效率为90.0%,对照组为78.3%,两组比较差异有统计学意义(P<0.05)。两组治疗后DHI各项评分及总分均较治疗前降低(P<0.05),治疗组治疗后生理评分、总分亦低于对照组治疗后(P<0.05或P<0.01)。治疗组治疗后SF-36健康评分除健康变化外,其他各项评分较治疗前差异均有统计学意义(P<0.05),治疗组治疗后躯体疼痛、精神健康评分与对照组比较,差异均有统计学意义(P<0.05)。结论整颈三步九法推拿治疗颈性眩晕可显著改善患者临床症状,提高生存质量。Objective To observe the therapeutic effect of Three-Step Nine-Therapy Cervical Manipulation（TNCM） for cervical vertigo.Methods Totally 120 cases of cervical vertigo were randomized into the treatment group and control group,with 60 in each.The treatment group was given TNCM and the control group was given traditional manipulation.Both groups were treated 20min/each time every other day and three times as a course of treatment.The therapeutic effect was observed after 2 courses of treatment.Dizziness Handicap Inventory（DHI）and the Short Form（36） Health Survey（SF-36） for quality of life（QOL） was scored before and after treatment.Results The total effective rate of the treatment group and control group was 90.0% and 78.3%respectively,with a significant difference between groups（P〈0.05）.The item scores and total score of DHI were significantly decreased after treatment in both groups（P〈0.05）,especially the physiological score and total score in the treatment group（P〈0.05 or P〈0.01）.There were significant differences in SF-36 scores except the health change score between before and after treatment in both groups（P〈0.05 or P〈0.01）.There were significant differences between groups in the scores of bodily pain and mental health after treatment（P〈0.05）.Conclusion TNCM can obviously relieve the symptoms and increase the QOL of patients with cervical vertigo.
