甘露聚糖肽联合阿德福韦酯治疗拉米夫定耐药的慢性乙型肝炎24例临床疗效

摘　　要：目的：观察甘露聚糖肽联合阿德福韦酯（ADV）治疗拉米夫定耐药的慢性乙型肝炎临床疗效。方法将24例拉米夫定治疗产生耐药的慢性乙型肝炎患者分为治疗组13例和对照组11例。其中治疗组给予甘露聚糖肽每日5～10mg，一日三次，同时口服阿德福韦酯10mg，每日一次，治疗时间为12个月。对照组给予口服阿德福韦酯10mg，每天一次，治疗时间为12个月。观察两组患者治疗12个月临床症状、肝功能（ALT）复常率、HBVDNA 转阴率、HBeAg 转阴率、HBeAg 血清转换率。结果治疗12个月，肝功能（ALT）复常率治疗组为76.9%，对照组为45.5%，两组比较，差异有统计学意义（P ＜0.01）。治疗组12个月 HBVDNA 阴转率、HBeAg 阴转率、HBeAg 血清转换率均显著高于对照组，两组比较差异有统计学意义（P ＜0.01）。且无不良反应。结论甘露聚糖肽联合阿德福韦酯治疗拉米夫定耐药的慢性乙肝能有效地改善肝功能，增强阿德福韦酯抑制乙肝病毒（HBV）复制。Objective To observe the effect of Mannatide combined with adefovir dipivoxil （ADV） clinical curative effect of the treatment of lamivudine resistant chronic hepatitis B. Chronic hepatitis B patients.Methods 24 patients with lamivudine in the treatment of drug resistance in 13 patients were divided into treatment group and control group with 11 cases. The treatment group was treated with mannan peptide daily 5 ~ 10mg, three times a day, and oral adefovir dipivoxil 10mg, once a day, treatment time is 12 months. The control group was treated with oral administration of adefovir dipivoxil 10mg, once a day, treatment time is 12 months. The two groups were observed for 12 months, the clinical symptoms, liver function （ALT） recovery rate, HBVDNA negative conversion rate, HBeAg negative conversion rate, HBeAg seroconversion rate. Results in 12 months of treatment, liver function （ALT） recovery rate was 76.9% in the treatment group, 45.5% in the control group, the two groups, the difference was statistically significant （P 〈 0.01）. The treatment group of 12 months HBVDNA negative conversion rate, HBeAg negative conversion rate, HBeAg seroconversion rates were significantly higher than those in control group, there was significant difference between two groups （P 〈 0.01）. And no adverse reaction. Chronic hepatitis B.Conclusion Mannatide combined with adefovir lamivudine resistance can effectively improve liver function, enhance the adefovir dipivoxil inhibition of hepatitis B virus （HBV） replication.
