摘　　要：目的：探讨105例神经外科术后病人进行系统的延续护理（家庭访视、电话随访、网络平台的健康教育等），其目的是为患者提供有效和低成本的健康服务。方法本组病例主要采用家庭访视护理中运用护理评估、诊断、实施、评价四个护理程序。内容包括服务方式、健康教育、心理护理、环境设置、功能锻炼、自理能力训练等。结果105例患者通过家庭访视提高了患者的生活自理能力，减轻经济负担，为患者提供了有效和低成本的健康服务。Objective Its objective is to investigate 105 cases of neurosurgical patients after extended care system （home visits, telephone follow-up, the network platform health education etc. Its purpose is to provide patients with an effective and low-cost health services. Its methods is that the patients used mainly in the use of nursing home visiting nursing assessment, diagnosis, implementation and evaluation of four nursing program [1]. Including service delivery, health education, psychological care, environment settings, functional training, self-care training. Its results is that 105 patients through home visits to improve the patient＇s daily living, reduce the financial burden for the patient to provide an effective and low-cost health services.
